摘要
目的观察康艾注射液联合去甲长春花碱（NVB，商品名盖诺）加顺铂（DDP）治疗晚期非小细胞肺癌（NSCLC）的临床疗效和不良反应。方法43例晚期NSCLC患者随机分为联合组（21例）和化疗组（22例），化疗组单纯采用NP方案化疗，NVB25mg／m^2，d1，8；DDP25—30mg／m^2，d1～3。21d为一周期。联合组在NP方案化疗基础上，同时应用康艾注射液40～60ml加入5％葡萄糖注射液或生理盐水250～500ml中静脉滴注，每日1次，连用21d。两个疗程后评价疗效。结果43例患者全部完成治疗计划，联合组总有效率（52．4％）高于化疗组（45．5％，P〉0．05），联合组1年生存率（47．6％）高于化疗组（40．9％，P〉0．05），差异均无显著性。联合组不良反应中自细胞下降发生率明显低于化疗组，差异有显著性（P〈0．05）。结论康艾注射液能提高NP方案的近期疗效，改善临床症状，减轻化疗的毒副反应，提高生存质量。
Objective To evaluate the efficacy and acute toxicity of combined chemotherapy with Kangai injection, navelbine（NVB） and cisplatin （DDP）in the treatment of the advanced non-small cell lung cancer（NSCLC）. Methods Forty-three patients with inoperable NSCLC were randomly divided into two groups ： combined chemotherapy group（ n = 21 ） and conventional chemotherapy group （ n = 22 ）. The patients in conventional chemotherapy group were treated with NP regimen（ NVB ＋ DDP） ：NVB 25mg/m^2 , d1, 8 and DDP 25-30 mg/m^2, d1-3. One cycle comprised 21 days. In the combined chemotherapy group, the patients were treated with NP regimen plus 40-60 ml Kangai injection every day for 21 days. The efficacy was evaluated after 2 cycles. Results All patients completed the treatment schedule. The overall response rate （ CR ＋ PR）in the combined chemotherapy group was higher than that in the conventional chemotherapy group： 52.4% vs 45.5% （P 〉 0.05 ）, and 1-year survival rates were 47.6% vs 40.9% （ P 〉 0.05 ） respectively, with no significant difference between the two groups. The rate of main toxicity including leukopenia in the combined chemotherapy group was lower than that in the conventional chemotherapy group, with significant difference（P 〈 0.05 ） . Conclusion The Kangai injection could enhance the short-term treatment efficacy of NVB and DDP in advanced non-small cell lung cancers, effectively meliorate the clinical symptoms and alleviate the side effects of chemotherapy, leading to improvement of the life quality of patients.
