重组人红细胞生成素在胸外科围术期应用的前瞻性研究 被引量:1

摘要 目的观察重组人红细胞生成素(rHuEPO)在普胸外科围术期对红细胞的动员效应。方法选择普胸手术患者80例,随机分为两组。实验组40例,从手术前4d至术后6d,皮下注射rHuEPO 10000IU/次,隔日1次,共6次;同时口服补充硫酸亚铁300mg,tid。对照组40例,仅给予铁剂,其他同实验组。术前4d(用药前)、1d、术后1d、4d、8d测血红蛋白(Hb)、红细胞压积(Hct)、网织红细胞(Ret)和红细胞(Rbc)。术前4d(用药前)及术后第8d测丙氨酸氨基转移酶(ALT)、总胆红素(TB)、尿素氮(BUN)。结果实验组Hb、Hct、Ret和Rbc计数均升高,术后8d分别为(124.12±7.27)g/L、(36.32±4.26)%、(28.14±7.24)%、(3.99±0.56)×1012/L,对照组为(117.67±10.76)g/L、(33.92±4.95)%、(23.76±10.08)%、(3.63±0.66)×1012/L,两组比较差异均有统计学意义(P<0.05)。而ALT、TB和BUN水平在用药前后则无明显变化。结论在胸外科围术期应用rHuEPO作为辅助治疗能有效地刺激动员红细胞。
