摘要
目的探讨妊娠合并白血病的处理及预后。方法回顾性分析妊娠合并白血病18例临床资料。结果①孕期多采用联合化疗。②妊娠结局:孕早期治疗性流产4例,中期引产7例,早产1例,足月分娩4例,产前死亡1例。③随访情况:失访2例,死亡4例,存活12例;5例新生儿,1例失访,4例正常。结论孕早期诊断急性白血病者应立即行治疗性流产并开始化疗;孕中晚期发病者可行联合化疗同时继续妊娠。影响预后最重要的因素是及时诊断和尽早化疗。
Objective Investigate the treatment and prognosis of leukaemia associated with pregnancy.Method Retrospectively analyze clinical data of 18 cases.Results ① Applying combined chemotherapy.② Pregnancy outcome：4 patients were ended with therapeutic abortion in first trimester,7 were induced abortion in second trimester,1 premature delivery,4 term delivery,1 died before delivery.③ Follow-up：2 patients lost,4 died,12 are alive;Of 5 newborn,1 lost,4 develop normally.Conclusion Therapeutic abortion should be offered to patients who develop AL during the first trimester;patients who developed leukaemia during the second and third semester can continue pregnancy while administering combined chemotherapy.The most important prognostic factor is to diagnose timely and outset intensive chemotherapy as early as possible.
