摘要
目的:研究急性前交叉韧带(ACL)断裂合并内侧副韧带(MCL)损伤时,内侧半月板(MM)、外侧半月板(LM)损伤的发生率及其相互关系。方法:1988年5月至2005年12月,对96例急性ACL断裂合并MCL损伤的患者进行膝关节镜检查,同时对合并的半月板损伤情况进行统计。按照MCL损伤程度分为Ⅰ度损伤组(5例)、Ⅱ度损伤组(21例)和Ⅲ度损伤组(70例);按照是否手术修补MCL分为手术组(51例)和非手术组(45例)。结果:在Ⅰ度、Ⅱ度和Ⅲ度损伤组中,MM和LM的损伤发生率分别为0和60%、4.8%和71.4%、17.1%和54.3%。在所有患者(手术组(非手术组)、非手术组和手术组中,MM和LM的损伤发生率分别为13.5%和58.3%、4.4%和62.2%、21.6%和54.9%。除Ⅰ度损伤组(P=0.05)外,其它各组中LM损伤发生率均显著高于MM(P<0.01)。结论:急性ACL断裂合并MCL损伤时,外侧半月板较内侧半月板更易受到损伤,这种三联损伤多是ACL、MCL和LM的联合损伤。
Objective To investigate the relation of the acute anterior cruciate ligament （ACL） rup- ture combined medial collateral ligament （MCL） injury to the menisci injury. Methods Ninety six pa- tients with acute ACL and MCL injuries were investigated by arthroscopy from May 1988 to December 2005. The associated injuries of menisci were observed and analyzed. Patients were divided into three groups according to the grade of MCL injury： group Ⅰ with grade Ⅰ MCL injury （5 cases）, group Ⅱ with grade Ⅱ MCL injury （21 cases） and group Ⅲ with grade Ⅲ MCL injury （70 cases）. Meanwhile, they were classified into two groups： operation group （51 cases） and conservative group （45 cases）. Results Incidence of the medial meniscus （MM） injury and lateral meniscus （LM） injury in the 5 groups were 0% and 60% （group Ⅰ ）, 4. 8~ and 71.4% （group Ⅱ ）, 17.1.% and 54.3% （ group Ⅲ）, 4. 4% and 62.2% （operation group）, 21.6% and 54. 9% （conservative group）, respectively. Among all the patients, the incidence of MM and LM injuries was 13.5% and 58.3%, respectively. The difference in the injury incidences of MM and LM in the 5 groups had been proven （P〈0.01）, except in grade Ⅰ （P=0. 05）. Conclusion Acute ACL injury was usually associated with the injuries of MCL and menisci. The trilogy of knee joint injury usually included combined injury of ACL, MCL and LM.
