直接冠状动脉支架置入术在老年冠心病的应用 被引量：1

摘要 目的观察直接冠状动脉支架置入术治疗老年冠状动脉狭窄的效果。方法87例患者随机分为治疗组(无球囊预扩张直接支架置入术)和对照组(在球囊预扩张的基础上行支架置入术),比较两组的支架置入成功率、并发症发生率、随访结果、平均手术时间等指标。结果两组患者支架放置成功率均为100%;住院并发症治疗组(3例)与对照组(10例)比较差异有统计学意义(P<0.05),1个月内再次介入或冠状动脉旁路移植术治疗组(1例)与对照组(7例)比较亦有差异有统计学意义(P<0.05);治疗组的平均手术时间、X线照射时间和住院时间明显短于对照组,造影剂用量也明显少于对照组,差异均有统计学意义(P<0.01)。全部患者随访24个月,对照组2例因急性心肌梗死死亡,3例再狭窄;治疗组1例行再次介入术。结论在选择合适病例的条件下,直接支架植入术在老年冠心病患者的治疗中疗效优于球囊扩张后支架植入术。
