摘要
目的探讨上颌窦不同部位病变的内镜辅助路径选择应用。方法回顾分析我科2003-11～2006-04间在我科住院治疗,并经CT和上颌窦内镜诊断上颌窦病变12例患者的临床资料,总结各壁相清应的附加路径。结果本组12例中上颌窦囊肿4例,上颌窦息肉5例,内翻性乳头状瘤3例经术后随访和复查3～26个月,未见复发。结论(1)联合中鼻道及下鼻道开窗适用于上颌窦外侧壁、后壁、前壁,以及上颌窦开口附近病变。(2)柯-陆式小切口联合下鼻道开窗适用于上颌窦底壁基底壁与内壁交界部病变。(3)柯-陆氏扩大切口并上颌窦内侧壁与下鼻甲后端部分切除适合于上颌窦广泛病变。
Objective To study the application in selection of additional bypass in endoscopic surgery in antrum maxillae diseases. Methods we retrospective analyzed clinical information about patients of antrum maxillae diseaseso who were diagnosised by CT and antrum maxillae endoscopic surgery in my depaurment and summarize corresponded additional bypass in every parietes. Results There are 12 antrum maxillae maxillae patients, 5 antrum maxillae polyp patients and 3 enstrophe corpora mammillaria tumor patients who have no recrudescence in 3 - 26 months after operation. Conclusion （1） Windowing in middle nasal meatus and inferior nasal meatus suitable to disease which near by lateral paries , aboral paries, ante paries and debouchement in antrum maxillae. （2） Windowing in inferior nasal meatus with K - Ls＇ small cut suitable to disease which in juncture of basis paries and inner paries in antrum maxillae. （3） Resection of inner paries in antrum maxillae and posterior segment of inferior nasal concha with K - Ls＇big cut suitable to.disease which disease which widespread in antrum maxillae.
