摘要 本文以D-二聚体水平及纤溶酶原活性为指标观察了40例脑血栓及23例脑出血病人的血浆纤溶状态。脑血栓组D-二聚体水平呈规律性变化,自病后48h内始显著上升,3~4d达高峰,以后渐下降,至1月时仍高于正常对照组;纤溶酶原活性于急性期(<1周)明显下降,病后3~4d降至最低值。D-二聚体水平达高峰时与纤溶酶原活性下降呈负直线相关。脑出血后D-二聚体水平于1月内亦显著增加。 To assess the time course of fibrinolysis after acute stroke,we measured plasma concentrations of D dimer and activities of plasminogen in 40 patients with cerebral thrombosis and 23 patients with cerebral hemorrhage at various times.D dimer levels were markedly elevated in both ischemic and hemorrhagic stroke within 48 hours of onsets(P<0.001 and P<0.01,respectively).In cerebral thrombosis,D dimer concentrations continuously increased to the peak values within the first week (P<0.001),whereas in cerebral hemorrhage the peak appeared at the end of the secone week.D dimer were still maintained significantly highy levels in both kinds of stroke patients until one month after stroke (P<0.02).Plasminogen activities decreased significantly only within the first week after ischemic stroke.
