摘要
目的:探讨儿童先天性胆管囊肿的 CT 诊断。材料与方法:40例儿童先天性胆管囊肿中,平扫40例,增强25例,胆系造影CT(CTC)10例。全部病例均作 B 超。40例中,男16例,女24例,年龄3天～11岁(平均3岁)。39例经手术和病理证实,1例经 B超、CT 和肝肾动脉造影确诊。结果:(1)肝外胆管囊肿35例,表现为肝门至胰头间圆形或椭圆形近水样低密度灶,增强无强化,CTC 示高密度造影剂进入囊腔内。邻近组织和器官不同程度受压;(2)肝内胆管囊肿(即 Caroli's 病)1例,示肝内胆管呈"串珠状"、"分节状"低密度灶,增强无强化,有中心点征。合并双侧髓质海绵肾;(3)肝内、外胆管囊肿4倒,除上述征象外,肝内、外同时发病是其特点。结论:儿童先天性胆管囊肿的 CT 表现具有一定特征性,尤其是 CTC 具有十分可靠的定性诊断价值。
Objective:To investigate the CT diagnostic criteria in children's congenital choledochal cysts.Materials and Methods: CT scan was performed in 40 cases with children's congenital choledochal cysts,25 of them underwent scanning after contrast injec- tion,and 10 underwent cholangiographic CT(CTC).Ultrasonography was perfolmed on all 40 cases.These patients ranged in age from 3 days ro 11 years(mean age 3 years).16 were males and 24 were females.39 cases were proved by surgery and pathology, 1 case was proved by ultrasonography.CT and hepatic-renal angiography.Results:The findings were(1)35 cases were extrahep- atic choledochal cysts.which showed round or elliptic water-like low density foci from hepatic porta to the head of pancreas.There were no enhancement after contrast injection.High-density contrast media were found in cyst cavity by CTC.Adjacent tissues and organs were compressed to different degree.(2)1 case was intrahepatic choledochal cyst(i.e.Caroli's disease),which showed 'beaded''segmental'low density foci,with no enhancement after contrast injection.There was central piont sign.It was accom- panied by bilateral medullary sponge kidneys.(3)4 cases were mixed intra-and extra hepatic choledochal cysts.Besides the above manifestations,it was characterized by simaltaneous occurance both inside and outside the liver.Conclusion:The CT manifesta- tions.especially CTC,are very reliabie in diagnosing the nature of children's congenital choledochal cysts.
