摘要
目的研究评价孟鲁司特钠与吸入布地奈德治疗儿童哮喘的疗效与不良反应。方法选择本院哮喘专科门诊和住院部儿童哮喘78例，随机分成治疗组39例，对照组39例，治疗组在常规吸入布地奈德400μg·d^-1基础上加用孟鲁司特钠，2-5岁4 mg·d^-1，5-14岁5 mg·d^-1，每晚顿服，共12周，并随访12周。观察期间如有哮喘急性发作，可加用β2受体激动剂。对照组39例，除不用孟鲁司特钠治疗外，其余治疗均与治疗组相同。结果在观察期间，治疗组平均每月日间哮喘症状发作次数、夜间发作次数、用短效支气管扩张剂的天数较对照组明显减少，差异有统计学意义（P〈0.01）。治疗组肺功能第一秒用力呼气容积（FEV1）及最大呼气流量峰值（PEF）的改善率均较对照组显著，差异有统计学意义（P〈0.05），不良反应较轻。结论孟鲁司特钠与吸入皮质激素联合治疗儿童哮喘具有叠加抗炎作用，其疗效优于单纯吸入激素，可作为治疗儿童哮喘的一种有效药物。
OBJECTIVE To research and evaluate the curative effect and side effects of therapeutic alliance for childish asthma with montelukast sodium and budesonide. METHODS Seventy-eight children with asthma was collected from asthmati outpatient department and the department of pediatric of Kaiping central hospital from 2003 to 2005, and assigned into treatment group 39 cases and control group 39 cases. The treatment group were treated by montelukast sodium 4 mg·d^-1 （ two years to five years）, 5 mg·d^-1 （ five years to fourteen years） take orally once every night in addition treat by imbibing budesonide 400 μg·d^-1. The control group were treated only by imbibing budesonide 400 μg·d^-1. Period of treatment was 12 weeks, and followed the tracks of patient for 12 weeks. During the observing, imbibing 132 was further given as asthma spasm. RESULTS In the treatment group, the times of asthma spasm and dyspnea in the night and the days, and the times of using short effective 132 adrenoceptor agonist were less than in the control group, there were marked difference between two groups （P 〈 0.01 ）. In the treatment group, the rate of ameliorated FEV1 and PEF were better than those in the control groups, there were marked difference between two groups （ P 〈 01 05 ）, and the adverse effects were reduced. CONCLUSION Montelukast sodium connecting with budesonide curing childish asthma have powerful affect, its effect is better than that only by imbibing glucocorticoid. It can become a good method curing the childish asthma.
