残余肾功能和透析充分性对腹膜透析患者血清脂联素水平的影响 被引量:2

摘要 目的探讨残余肾功能和透析充分性对腹膜透析患者血清脂联素水平的影响。方法选择上海交通大学医学院附属仁济医院肾内科59例腹膜透析患者进行透析充分性,残余肾功能和腹膜转运特性的检查,并采用酶联免疫复合物法(ELISA)测定血清脂联素水平。结果与正常对照组相比,腹膜透析组患者的血清脂联素水平显著升高。血清脂联素水平与腹膜透析患者的总肌酐清除率(Ccr)、总Kt/V、腹膜Ccr、腹膜Kt/V、残存肾Kt/V、残存肾Ccr和残肾肾小球滤过率(GFR)无显著相关性。血清脂联素水平与腹膜转运类型也无显著相关性。腹膜透析患者血清脂联素水平与C反应蛋白、白介素-6、体重指数(BMI)、三酰甘油呈负相关,与高密度脂蛋白呈正相关。结论腹膜透析患者血清脂联素水平与透析充分性,残余肾功能以及腹膜转运特性未见统计学相关性,而与其炎症状态,BMI和脂代谢情况有关。 Objectives To observe serum adiponectin (ADPN) in relation to residual renal function and dialysis adequacy in peritoneal dialysis patients. Methods Residual glomerular filtration rate, Kt/V and creatinine clearance (Ccr) of peritoneal dialysis and Kt/V and Ccr of residual kidney were estimated in 59 continuous ambulatory peritoneal dialysis (CAPD) patients treated in this hospital. Serum ADPN level was measured by ELISA. Results Serum ADPN was higher in CAPD patients than in healthy subjects (13.09±7.54μg/ml vs 6.65±4.33μg/ml, P〈0.01). SerumADPN did not correlate to total Ccr and Kt/V, nor to residual renal Ccr and Kt/V, peritoneal Ccr and Kt/V and residual kidney glomerular filtration rate. There was also no relationship between serum ADPN and peritoneal transport type. Serum ADPN inversely related to body mass index, triglycerides, C-reaction protein and interleukin- 6, and positively related to high density lipoproteins (P 〈0.01) of the patients. Conclusions Serum ADPN is influenced by inflammation, body mass index and lipids metabolism, but not related to residual renal function, dialysis adequacy and peritoneal transport type of the patients.
