摘要
目的比较肌力训练结合常规康复治疗与常规康复治疗对痉挛型脑瘫患儿运动功能的临床疗效差异。方法58例脑瘫患儿随机分为两组,即肌力训练结合常规康复治疗组(肌力训练组,30例)和常规康复治疗组(28例)。两组患儿入组后开始为期2周的康复治疗,2周后改由家长在家进行治疗。分别在治疗前,治疗后的6周、12周进行以下评定:(1)综合痉挛量表(CSS);(2)粗大运动功能量表(GMFM)中的D和E两项;(3)步行速度。结果两组患儿的CSS、GMFM评分及步行速度治疗前差异无显著性意义(P>0.05),治疗后6周、12周与治疗前比较差异均有显著性意义(P<0.05),GMFM评分、步行速度治疗后6周、12周两组比较差异均有显著性意义(P<0.05)。结论肌力训练结合常规康复治疗能更好地提高痉挛型脑瘫患儿的运动能力。
Objective To investigate the differences in efficacies between strength training in combination with routine rehabilitation and the sole application of routine rehabilitation for the improvement of motor function in children with spastic cerebral palsy （SCP）. Methods A total of 58 children with SCP were randomly assigned into 2 groups, namely, the group of strength training in combination with routine rehabilitation （strength training group, 30 cases） and the group with the sole application of routine rehabilitation （28 cases）. Children in both groups received two-week rehabilitation treatment since they were assigned, and after the first two weeks, they were administrated by patients at home. Clinical assessments were performed before the treatment and at the 6th and 12th week after the treatment, including： （1） composite spasticity scale （CSS）; （2） D and E dimensions of the gross motor function measure （GMFM）; （3） walking velocity （WV）. Results There were no significant statistical differences in CSS scores, GMFM scores and WV before the treatment of the two groups （P〉0.05）, but differences were statistically significant before the treatment and at the 6th and 12th week after the treatment （P〈0.05）; differences of GMFM scores and WV at the 6th and 12th week after the treatment were significant statistically （P〈0.05）. Conclusion Strength training in combination with routine rehabilitation may enhance the motor function of children with SCP more effectively than the sole application of routine rehabilitation.
