摘要
目的观察视神经减压术治疗创伤性视神经病的临床疗效。方法30例创伤性视神经病患者,分别采用鼻外径路或经鼻内镜筛蝶窦径路施行视神经减压术,观察手术治疗的临床效果。结果总有效率56.67%。术前无光感的16例中,6例术后视力显示一定程度的改善;术前有残余视力的14例中,11例的术后视力均有不同程度的提高。无1例发生严重并发症。结论视神经减压术对创伤性视神经病导致的视力障碍,无论伤后时间长短和是否具有残余视力,均具有确切疗效。
Objective To investigate the therapeutic effect of optic nerve decompression on traumatic optic neuropathy based on a clinical observation on a group of cases with such a lesion. Methods Included in this study were 30 cases with traumatic optic neuropathy, treated by medication plus optic nerve decompression surgery, either via the extra-nasal approach or via trans-ethrnoid and sphenoid approach under nasal endoscope. Then, all the patients were followed up for a period of time lasted for I month to I year following the surgery to evaluate their therapeutic response. Results The total effective rate was 56.67 % for this group of cases, with no postoperative complications occurred after the operation. Only 6 showed some degrees of visual acuity improving among the 16 cases with no light perception preoperatively, while 11 were seen with various degrees of visual acuity improving, varied respectively from light perception, or counting finger, to finger movement, or 0.6 in the visual acuity, among the 14 cases with residual vision preoperatively. Conclusions Optic never decompression is surely effective for the treatment of traumatic optic neuropathy, no matter the length of post-trauma period and the degrees of residual visual acuity following the trauma.
