经颅多普勒和脑干听觉诱发电位对椎基底动脉供血不足的诊断价值 被引量:8

The diagnostic value of TCD and BAEP for patients with vertebrobasilar insufficiency

摘要 目的研究经颅多普勒(TCD)与脑干听觉诱发电位(BAEP)联合检查对椎基底动脉供血不足(VBI)的诊断价值。方法对研究组102例患者和正常对照组60例进行TCD和BAEP检测。观察比较2组各项检查参数的变化及其之间的相关性。结果TCD显示研究组左右椎动脉和基底动脉血流速度均低于对照组且血管搏动指数增高,差异有非常显著意义(P<0.01)。BAEP显示研究组V波峰潜伏期(PL)和I~III、III~V、I~V峰间潜伏期(IPL)及(III~V)/(I~III)比值均高于对照组,差异有非常显著意义(P<0.01)。左右椎动脉和基底动脉血流速度与V波PL和I~V波IPL呈负相关(P<0.05)。研究组TCD的异常率为80.39%,BAEP的异常率为82.35%,两者比较差异无显著意义(P>0.05)。结论TCD和BAEP联合检测可作为VBI的一种简便、无创、准确的诊断方法。 Objective To investigate the diagnostic value of transcranail Doppler (TCD) and brain stem auditory evoked potential (BAEP) for patients with vertebrohasilar insufficiency (VBI). Methods TCD and BAEP were performed for 102 patients in observation group and 60 healthy subjects in control group. Results TCD showed that Vp of LVA, RVA and BA were significantly lower and PI was obviously higher in observation group than those in control group ( P 〈 0.01 ). BAEP showed that PL of Ⅴ wave, Ⅰ- Ⅲ, Ⅲ - Ⅴ, Ⅰ- Ⅴ IPL and Ⅲ - Ⅴ/Ⅰ - Ⅲ ratio were significantly higher in observation group than those in control group ( P 〈 0.01 ). The Vp and PI of LVA, RVA, BA were negatively correlated with PL of Ⅴ wave and Ⅰ- Ⅴ IPL ( P 〈 0.05). The abnormality rate of TCD was 80.39% and 82.35 % for BAEP in observation group, there was no significant difference ( P 〉 0.05). Conclusion BAEP and TCD are simple, noninvasive and exact methods for the diagnosis of VBI.
