摘要
通过观察肝硬变门静脉高压大鼠在行门腔静脉分流(PCS)术后胃粘膜的功能改变。结果发现门静脉高压时胃窦及底体部粘膜血流量(GMBF)、糖蛋白(GP)及前列腺素(PGE_2、PGI_2)含量等均显著下降,PCS术后上述指标则明显升高,尤其是GMBF在术后5周时已接近于正常水平。结果表明,肝硬变门静脉高压时胃粘膜的微循环及防病能力被削弱,导致了胃粘膜病变的发生;PCS术后由于降低了门静脉系统压力,改善了粘膜下静脉的瘀血状态,增加了粘膜层的血流量,同时提高了粘膜上皮合成及分泌GP、PGs等的能力,提高了粘膜抗病力,对治疗门静脉高压性胃粘膜病变有一定意义。
The changes of gastric mucosa in rats with portal hypertension after portacaval shunt(PCS) were observed. It was found that the levels of the gastric mucosal blood flow(GMBF), blycoprotein(GP)and prostaglandins(PGE and PGI_2) of the gastric corpus and antrum were significantly decreased, while they were significantly increased after PCS, especially GMBF restored to the normal level on the fifth week postoperatively. Our findings suggest that: 1). The microcirculation and defensive capacity of the gastric mucosa under the cirrhotic portal hypertension is weakened to result in pathological changes of the gastric mucosa; 2). PCS decreases the portal pressure, alleviates the submucous venous ectasia and increases the volume of GMBF. Meanwhile, it improves the function of the mucosa to synthesize and secret GP and PG and therefore the defensive capacity of the gastric mucosa is strengthened. PCS may play a important role in treating the pathological processes of the gastric mucosa because of the portal hypertension.
