摘要
目的探讨三维(3D)薄层动态增强MRI检查对肝门胆管癌显示、分型的技术优势及各型肿瘤的MRI表现。资料与方法搜集19例(6例肿块型,11例浸润型,2例内生型)经手术病理证实的肝门胆管癌MRI资料。所有患者术前接受了上腹部MRI检查,包括2DT1WI、T2WI平扫,2D磁共振胰胆管成像(MRCP),3D薄层三期动态增强扫描和2DT1WI平衡期增强扫描。分别观察3D薄层动态增强MRI图像、2DT1WI增强扫描平衡期图像和MRCP图像3种MRI序列对肿瘤的显示及分型情况。结果3D薄层动态增强MRI检查序列能清晰显示19例患者的肿瘤并正确分型;2DT1WI增强序列未能显示4例浸润型肝门胆管癌的瘤体情况(21.1%),MRCP仅能判断1例浸润型肿瘤的分型(5.3%)。结论3D薄层动态增强MRI检查能准确显示肝门胆管癌及其分型,特别是对浸润型肿瘤的显示具有较大优势。
Objective To investigate the diagnostic value of a three-dimensional (3D) thin-section dynamic enhanced MRI sequence in the detection and morphological classification of hiLar cholangiocarcinoma, and to describe their MRI manifestations. Materials and Methods MRI data of 19 patients with hilar cholangiocarcinoma (6 mass-forming type, 11 periductal-infiltrating type, and 2 intraductal-growing type) confirmed by surgery and pathology were retrospectively analyzed. Comprehensive MRI examination was performed for all patients, sequences including 2D T1W and T2W, 2D MRCP, 3D thin-section dynamic enhanced tri-phase scanning, and 2D T1W enhanced scanning at equilibrium phase. The depiction of the tumors, as well as the morphological classification of hilar cholangiocarcinoma were compared with 3D thin-section dynamic enhanced MR images, 2D T1W enhanced images and MRCP images respectively. Restdts 3D thin-section dynamic enhanced MRI images directly displayed hilar tumors in all patients and correctly classified their morphological types, while 2D T1W sequence missed 4 patients of periductal-infiltrating type (22.1% ), and MRCP could only classify the morphological type of hilar cholangiocarcinoma in 1 periductal-infiltrating type patient (5.3%). Conclusion 3D thin-section dynamic enhanced MBI sequence is very valuable in both the depiction and morphological classification of hiLar cholangiocarcinoma, especially in the periductal-infiltrating type.
