乳癌组织中Cyclin D_1的表达及临床意义 被引量:1

摘要 目的探讨细胞周期蛋白D1(Cyclin D1)对乳癌诊断及预后判断的意义。方法采用免疫组织化学SP法对78例乳腺浸润性导管癌Cyclin D1表达进行检测,并结合雌激素受体(ER)表达情况,对两者关系以及预后进行分析。以37例乳腺腺病和乳腺不典型增生作为对照。结果对照组及乳癌组Cyclin D1阳性表达率分别为18.92%和55.13%,差别有统计学意义(2χ=7.291,P<0.01)。Cyclin D1阳性率在乳癌高分化组为47.62%,中分化组为61.76%,低分化组为17.39%,各组间差别有显著统计学意义(2χ=11.008,P<0.05)。ER阳性乳癌病人Cyclin D1阳性表达率高于阴性病人,差别有统计学意义(χ2=4.019,P<0.05)。Cyclin D1(+)ER(+)组预后与Cyclin D1(+)ER(-)及Cyclin D1(-)ER(-)组比较差别有统计学意义(χ2=4.501、3.992,P<0.05),而与CyclinD1(-)ER(+)组比较差别无统计学意义。结论Cyclin D1的表达是肿瘤发生中的早期事件,参与癌前病变的发生并影响癌细胞分化程度。Cyclin D1在乳癌中表达与病理分级、ER状态有关。Cyclin D1异常表达的ER(-)乳癌病人预后较差,而Cyclin D1异常表达的ER(+)乳癌病人预后最好。 Objective To investigate the expression of cyclin D1 in breast cancer and to evaluate its clinical significance in predicting its prognosis. Methods Cyclin D; was measured in 78 cases of breast cancer tissues and 37 cases of benign disease by immunohistochemistry SP method. The results were analyzed and compared. Results The positive rate of cyclin D; in breast cancer in different pathologic stages was 47. 62% (G1), 61.76% (G2) and 17. 39% (G3), respectively. In benign disease, it was 6/17 (35. 29%, atypical ductal hyperplasia) and 1/20 (5.00%, common breast disease). The difference was significant between the cancer and benign one. The expression of cyclin D1 in the cancer was significantly higher than that in benign disease (X^2 = 7.291, P〈0.01 ). Their expression was related to pathologic stage (X^2 = 11. 008, P〈0.01). Analysis of the relationship of cyclin D; and estrogen receptor (ER) showed that the positive rate of cyclin Dt in ER (+) tissue was higher than ER(- ) tissue (X^2 = 4. 019, P〈0.05). The difference between the groups of Cyclin D1 (+) ER (+) and cyclin D; ( + ) ER ( - ) was significant (X^2 = 4. 501,P〈0.01). Conclusion Overexpression of cyclin D1 plays an important role in the differentiation and development of cancer. The expression of cyclin D1 in breast cancer is related to pathologic stages. The high expression of cyclin D; protein and positive expression of ER is highly identical in breast cancer. Overexpression of cyclin D1 predicts poor prognosis in ER-negative pa- tients and good prognosis in ER- positive.
