川崎病患儿血清C反应蛋白测定及意义 被引量:7

The Clinical Significance of Serumal C-Reactive Protein Count in Children with Kawasaki Disease

摘要 目的 检测川崎病(KD)患儿CRP水平变化,探讨测定川崎病患儿CRP值的临床意义。方法 分别于静脉用丙种球蛋白(IVIG)前后测定2OO3年1月~2006年5月我院收治的30例川崎病患儿的白细胞,CRP,血沉,彩色超声心动图,对照组2O例,均为与川崎病患儿年龄,性别相匹配的健康儿童,测定其CRP,WBC值。结果 30例KD患儿在IVIG治疗前CRP值为(75.90±24.5)mg/L,对照组CRP值(3.91±2.24)mg/L,有显著性差异(P〈0.01),30例KD患儿合并冠脉扩张8例,无一例形成动脉瘤。合并冠脉异常8例,其CRP值与冠脉正常组相比差异无显著性(P〉0.05),静脉应用丙球后冠脉正常组迅速下降至正常,而冠脉异常组CRP值下降不明显,有显著性差异(P〈0.01),其中1例对静脉丙球无反应,在治疗后CRP仍持续增高。结论 KD患儿血清CRP是增高的,CRP可作为KD患儿的一个诊断指标。动态观察CRP水平有助于对KD患儿并发冠脉损伤及静脉丙球无反应的早期判断。 Objective To investigate the clinical significance of the variability of serumal C-reactive protein level in children with Kawasaki disease. Methods The counts of white corpuscles, CRP, ESR and color echocardiographic before and after treated with intravenous immunoglobulin of 30 cases of Kawasaki Disease from Jan. 2003 to May. 2006 in Paediatrics of Xinhui People's Hospital compares with the counts of CRP and white corpuscles in healthy children of control group of 20 cases. There were no significant differences in gender proportion and age in two groups. Result There was significant difference (p〈0. 01) of CRP level in untreated children with Kawasaki Disease and healthy children. The counts of CRP were (75.90±24.5) mg/L and (3.91±2.24) mg/L respectively. There were 8 cases with coronary arteries dilation and none with aneurysm in observational group. There were no significant differences (P〉0. 05) of CRP level in children with coronary artery lesions and in children with normal coronary arteries in observational group. The CRP level in children with normal coronary arteries in observational group jumped to a normal level after treated with intravenous immunoglobulin, but the stagnant level of CRP in children with coronary artery lesions appeared, and statistical analysis revealed that there was significant difference (p〈0. 01). There was no curative effect in one case in which the count of CRP was continue to increase after treated with intravenous immunoglobulin. Conclusion The high level of CRP should be a diagnosis index of Kawasaki Disease in children. The developmental tendency of CRP level is a predictor of coronary artery lesions and reaction of the treatment with intravenous immunoglobulin in children with Kawasaki Disease.
