从骨折的机械固定到生物学固定 被引量:5

摘要 近年,骨折的治疗原则发生了很大的变化,由机械力学固定(AO)体系逐渐转向生物学固定(Biological osteosynthesis,BO)体系。早期传统机械固定主要用牵引或石膏固定的方法以达到骨折复位和制动,但治疗周期长且存在畸形愈合及功能障碍。到上世纪中期,逐渐兴起AO骨折固定体系,即绝对的解剖复位、坚强内固定及早期功能锻炼。为了达到解剖复位和坚强固定,常需要骨折部位充分的暴露,使骨膜及软组织遭到破坏。影响了骨折部位的血供,局部组织的生物学活性遭到严重损伤,导致骨不愈合或延迟愈合。至上世纪后期,骨折的治疗重点逐渐从AO转向BO,主要强调微创或无创外科技术。使骨折端血运得以保护,有活力的骨折块迅速生长,最终得到良好愈合。
