应用简易葡萄糖处置指数评估多囊卵巢综合征患者胰岛β细胞功能 被引量:1

摘要 目的探讨应用通过OGTT获得的简易葡萄糖处置指数(DI=ΔI30/ΔG30×1/HOMA-IR)评估多囊卵巢综合征(PCOS)患者胰岛β细胞功能的可能性。方法79例PCOS患者根据OGTT结果分为正常糖耐量(NGT)组(47例)和糖耐量减低(IGT)组(32例),分别行Botnia葡萄糖钳夹术,该钳夹术包括静脉葡萄糖耐量试验和高胰岛素-正糖钳夹术,DI=胰岛素敏感性×胰岛β细胞功能。结果(1)ΔI30/ΔG30×1/HOMA-IR与从钳夹术获取的DI呈正相关(NGT组r为0.51,P≤0.001;IGT组r为0.53,P<0.05)。(2)ΔI30/ΔG30×1/HOMA-IR与OGTT各时点血糖浓度呈负相关(P<0.05),且多元回归分析发现其对2hPG影响最大(回归模型P<0.0001)。结论ΔI30/ΔG30×1/HOMA-IR与Botnia葡萄糖钳夹术获取的DI呈正相关,线性相关性优于其他胰岛β细胞功能简易指数,可作为反映胰岛β细胞功能简单、有效的指标。 Objective To explore whether simple glucose disposition index (△I30/△G30 × 1/HOMA-IR)obtained from OGTT is reliable for evaluating islet β-cell function in PCOS patients. Methods 79 subjects were given oral glucose tolerance tests (OGTT) and Bomia glucose clamp which was an intravenous glucose tolerance test (IVGTT) was followed by an euglycemic-hyperinsulinemic clamp. The subjects were divided into NGT and IGT group(47 vs 32 subjects). Glucose disposition index (DI) was calculated as the product of IS and βF (DI= IS×βF). Results (1) △I30/△G30 × 1/ HOMA-IR showed positive linear correlation with DI (AIR × Clamp-IS)obtained from Botnia glucose clamp(NGT, r= 0.51, P≤0. 001 ; IGT, r= 0.53, P〈0. 05). (2) △I30/△G30 ×1/HOMA-IR showed inverse linear correlation with the glucose level at 0, 30, 60 and 120min of OGTT (P〈0. 05), and it was the most significantly independent variable for 2hPG in stepwise regression analysis. Conclusions △I30/△G30 × 1/HOMA-IR shows better positive linear correlation with DI obtained from Botnia glucose clamp than other β-cell function indexes, so it is simple and reliable index for evaluating β-cell function in PCOS patients.
