摘要
目的探讨先天性肌性斜颈病变组织中细胞凋亡的程度。方法通过原位末端标记法检测组织中细胞的凋亡情况,采用TUNEL法检测,用光镜观察凋亡细胞,电镜观察超微结构。结果本组42例,男30例,女12例。手术时19例能扪及肿块,23例不能扪及肿块(其中15例有肿块史)。年龄<3个月7例;3~8个月17例;9~12个月7例;>12个月11例。光镜发现一些细胞核染色深。电镜发现肌母细胞核异染色质边积;成纤维细胞髓样结构形成,核固缩;肌细胞肌丝溶解。TUNEL法显示,在有肿块的病儿和月龄3~12个月病儿中,细胞凋亡发生明显。结论先天性肌性斜颈病变组织中不同细胞之间凋亡发生程度不同,可能与疾病的转归及愈后有关。
Objective To seek evidence apoptosis in pathologic tissues for sternoleidomastoid pseudotumor of infancy (SCMPOI) and congenital muscular torticollis (CMT). Methods The specimens from forty-two cases were investigated by light and electron microscope and TdT mediated dUTP nick end labeling technique for in situs cell death detection. Of 42 cases, 19 were with phymata and 33 without. The age distribution were as follow: 〈3 months, 7 cases; 3-8 months, 17 cases; 9-12 months, 7 cases; and 〉 1 year, 11 cases. Statistic procedure: all cases were grouped by age and tumor conditions. The apoptosis cells were recorded by x ± s in different groups and t-test was used for the significance in each group. Results By HE stain, all cases showed that there were numerous cell with nuclear pyknosis, myofilament collapse eosinophilic cytoplasm distributing in the proliferation interstitium. By electron microscope, we find that some myoblasts showed numerous large vacuoles, margination of heterochromatin and breakdown of cell membrane. Some fibroblasts showed karyopyknosis and myelin figure. Muscle showed myelin figure, myofibril lysis and bulla formation. Terminal deoxynucleotidy transferase biotin nick end-labeling (TUNEL) techniques determined results showed that cell death more significantly in patients with phymata or in patients during 3-12 months age. Conclusions The apoptosis may have masked important rule in the basic pathology of SCMPOI and CMT. For SCMPOI, apoptosis might induce the phymata disappeared. For CMT, the apoptosis might induce fibrosis of SCM. As proliferation of myoblasts, myofibroblasts and fibroblasts were found in the interstitium of SCMPOI, if the SCMPOI could spontaneous cure depend on which cell happen to apoptosis most.
