机械通气治疗危重哮喘16例 被引量:6

摘要 目的:探讨应用机械通气治疗危重哮喘的临床疗效。方法:16例患者均采用同步间歇指令通气与呼气末正压通气,吸入氧浓度0.4~1.0,潮气量5~10ml/kg,呼吸频率10~15次/min,吸呼比为1∶2~3,气道压力在50cmH2O以下,平台压在36cmH2O以下,同时记录气道峰压(PIP),动脉血气(ABG),血压(BP)和心率(HR)。结果:机械通气后,血pH值、PO2均恢复至正常(P<0.01),PaCO2在正常水平上限逐渐下降(P<0.01)。所有患者均存活,未发生并发症。结论:及早应用机械通气可有效治疗危重哮喘,是挽救生命,改善症状的有效疗法。 Objective:To explore the clinical effects of mechanical ventilation for patients with severe asthma. Methods: Sixteen patients underwent synchronized intermittent mandatory ventilation and positive end-expiratory pressure ventilation(SIMV + PEEP). FiO2 0. 4-1.0, tidal volume 5-10ml/kg, breath frequency 10-15 per minute, I/E 1:2~3, airway pressure less than 50cmH2O and the plat form pressure less than 36 cm H2O. Peak inspiratory pressure(PIP) ,artery Blood analysis(ABG),blood pressure and heart rate were recorded at the same time. Results: Blood pH and PaO2 showed significant increased(P〈0. 01)and the PaCO2 remained at upper normal limit or decreased gradually(P〈0. 01 ). All patients survived without any complication. Conclusion: Early application of mechanical ventilation on severe asthma is one of the effective methods to save patients' life and alleviates the symptoms.
