187株深部感染真菌的菌群分布及耐药性分析 被引量:4

摘要 目的了解外科病房检出深部感染真菌的菌群分布及耐药性状况,为临床治疗提供参考依据。方法采集外科各病区住院病人的血液、痰液、尿液引流液和大便等标本培养分离菌株。分离出的真菌用API20CAUX鉴定.用ATBFUNGUS进行药敏实验。结果2003年4月.2004年12月从外科病房检出真菌187株,居前3位为白色念株菌99株(53.0%)、热带念珠菌38株(20.3%)和光滑念珠菌26株(13.9%)。以痰液、尿液、大便3种标本的检出率较高。分别占45.4%、13.4%、10.2%。科室来源主要是SICU、器官移植外科、胃肠外科。真菌对5-氟胞嘧啶、两性霉素B、氟康唑、伊曲康唑总的敏感率分别为93.0%、98.9%、90.8%、59.8%。结论外科深部真菌感染主要由白色念株菌、热带念珠菌、光滑念珠菌引起,以呼吸道、尿道和消化道感染常见。两性霉素B、5-氟胞嘧啶、氟康唑对菌株的敏感性高,是治疗外科深部真菌感染的有效药物。 Objective To investigate the distribution and drug resistance of the fungi which caused deep infection in surgery ward. Methods The fungi were indentified with API 20C AUX and drug susceptibility test was conducted by ATB FUNGUS. Results 187 strains of fungi were isolated from Apr 2003 to Dec 2004. The infection of Canadida albicans predominated, 99 strains accounted for 53.0%, 38 strains(20.3% ) of Canadida tropicalis, 26 strains( 13.9% ) of Canadida glabrata were isolated. The samples were mainly sputun(45.4%), urine(13,4%), stool(10.2% ). SICU, department of organ transplantation and gastrointestinal surgery were mainly involved. The total sensitivity to 5- Fluorocytosine, Amphoteficin- B, Fluconazol and Itraconazole were 93% ,98.9% ,90.8% and 59.8% respectively. Conclusion Candida albicans, Candida tropicalis and Canadida glabrata are the main pathogens responsible for deep fungi infection in surgery, which dominated in respiratory tract, urine tract and digestive tract, and higldy sensitive in vitro to Amphoteficin- B, 5 - fluorocytosine and Fluconazol which are the effective drugs for treatment of deep fungus infection.
