江苏地区乙型肝炎患者HBV基因分型与血清HBV DNA水平研究 被引量:2

Study on the relationship between hepatitis B virus genotypes and HBV DNA levels in patients with hepatitis B in Jiangsu Province

摘要 目的了解江苏地区乙型肝炎病毒基因型分布特征,探讨HBV各基因型与血清HBV DNA水平、患者年龄之间的关系。方法采用荧光定量PCR结合Taqman MGB探针技术,对江苏地区176份乙型肝炎患者血清中的HBV DNA进行基因分型和定量检测。结果176例血清中,C型117人(66.5%),B、C型40人(22.7%),B型9人(5.1%),D型1人(0.6%),A型1人(0.6%)。C型与B、C混合型在HBV DNA水平上存在显著差异(t=2.413,P<0.05),B、C混合型的HBV DNA水平明显高于C型。C基因型在慢性乙型肝炎患者中的所占比例(71.4%)比在急性乙型肝炎、肝硬化和肝癌患者中所占的比例较高,但差异无显著性。HBV DNA各基因型在患者年龄水平上没有明显差异(P>0.05)。结论江苏地区HBV基因型以C型为主、B、C混合型次之,B型较少,D型、A型极少;B、C混合型患者的HBV DNA数量水平显著高于C型。 Objective To find out the distribution of hepatitis B virus genotypes in patients with hepatitis B and analyze the relationship between HBV genotypes and HBV DNA levels of the patients in Jiangsu province. Methods HBV genotypes and HBV DNA levels, from 176 patients in Jiangsu province, were detected by real time PCR and taqman MGB technique. Results Among 176 HBV-infeeted patients, the proportions of genotype C, B with C mix-type, B, A and D were 66.5%(117/176), 22.7%(40/176), 5.1%(9/176), 0.6%(1/176) and 0.6%(1/176), respectively. The results of HBV DNA load showed significant difference between genotype C and B with C mix-type(t=2.413, P〈0.05), and HBV DNA level of genotype B with C was significantly higher than that of genotype C. The rate of genotype C was 71.4% in patients with chronic hepatitis B, which was higher than that in patients with acute hepatitis B, liver cirrhosis and bepatoeellular carcinoma, but the results showed no significant differences among these groups(P,〉0.05). In addition, genotype distribution showed no significantly different between certain HBV genotype and the age of the patients(P〉0.05). Conclusions Genotype C and B with C mix-type are dominant genotypes in the patients, and genotype B, A, and D are also found in Jiangsu province. The HBV DNA load of genotype B with C is significantly higher than that of genotype C.
