尿中8-羟基脱氧鸟苷作为肿瘤疗效评价生物标记物的研究 被引量:10

Study on Urinary 8-hydroxydeoxyguanosine as a Biomarker of Evaluating the Effect of Therapy for Tumor Patients

摘要 目的探讨尿中8-羟基脱氧鸟苷(80HdG)作为肿瘤手术治疗和化疗效果监测与评价生物标记物的可行性。方法建立了两种测定尿中80HdG的分析方法:气相色谱/质谱法(GC/MS)和毛细管电泳法(CE),比较了二者的定量分析特性及其相关性。利用建立的GC/MS法检测了4例癌症患者和2例正常对照者尿中80HdG的排放水平;利用建立的CE法,测定了28例肿瘤患者和9例正常对照者尿中80HdG的排放水平,并监测手术前后、化疗前后患者尿中80HdG排放水平的变化。结果两种方法的定量分析特性都能满足临床分析的需要,且两者的相关性很好。肿瘤患者尿中80HdG的排放水平[(35.26±27.96)nmol/L]明显高于正常对照组[(13.51±5.08)nmol/L,P〈0.053,手术后尿中80HdG的排放水平[(21.34±14.17)nmol/L]与未手术[(41.78±19.96)nmol/L]相比显著减低(P〈0.05),与正常对照组相接近。化疗后2~6d患者尿中80HdG的排放水平[(97.13±31.24)nmol/L]显著高于化疗前[(34.45±21.46)nmol/L],而化疗后30d左右患者尿中80HdG的排放水平[(30.24±19.35)nmol/L]已经明显降低,略低于化疗前的排放水平。结论尿中80HdG有望作为肿瘤患者早期诊断和手术治疗效果评价及监测的生物标记物。 Objective To investigate urinary 8-hydroxydeoxyguanosine (8OHdG) as a biomarker of evaluating and monitoring the effect of surgical therapy and chemotherapy for tumor patients. Methods Two methods-capillary electrophoresis (CE) and gas chromatography/mass spectrometry (GC/MS) were developed for urinary 8OHdG analysis and compared. Using the developed GC/MS method, the urinary 8OHdG levels in 4 tumor patients and 2 controls were determined. By using the developed CE method, the urinary 8OHdG levels in 28 tumor patients and 9 controls were determined. Moreover, the urinary 8OHdG levels in some tumor patients before and after surgical therapy, the others before and after chemotherapy were investigated. Results Quantitative characteristics of two methods were both suitable for clinical application and the correlation was very good. It was found that the excretion levels of urinary 8OHdG in tumor patients were significantly higher than those in healthy persons [(35. 26±27.96) vs (13. 51±5.08) nmol/L, P〈0.05]. Tumor patients receiving surgical therapy showed a significant decrease in urinary 8OHdG [ (41.78 ± 19.96) vs (21.34 ± 14.17) nmol/L, P〈0.05] and the extraction levels of urinary 8OHdG had no significant difference between those patients after surgical therapy and healthy persons [-(21.34± 14.17) vs (13.51 ± 5.08) nmol/L]. As compared with the patients before chemotherapy, there was a remarkable increase of the excretion levels of urinary 8OHdG from the patients receiving chemotherapy after 2-6 days [(97.13±31.24) vs (34. 45±21.46) nmol/L]. However after 30 days following chemotherapy the patients showed a decrease of the excretion levels and a little lower than the patients before chemotherapy [(30.24± 19.35) vs (34.45±21.46) nmol/L]. Conclusion Urinary 8OHdG might be used as a useful biomarker for early diagnosing tumor, evaluating and monitoring the effect of surgical therapy.
