摘要 目的探讨团注追踪法64层螺旋CT增强扫描技术在主动脉成像中的应用价值。方法对39例拟诊为主动脉病变的患者采用团注追踪法CT增强扫描。以3.5~4ml/s速率分别注射100ml非离子型对比剂和40~50ml生理盐水。计算夹层组和非夹层组主动脉平均强化值的均数和标准差,并进行统计学比较。将两组的图像质量按优、良、差和失败进行分级并行比较。结果非夹层组与夹层组真腔的主动脉平均强化值比较,差异在统计学上无显著性意义(P〉0.05);与夹层组假腔的主动脉平均强化值比较,差异有显著性意义(P=0.01)。夹层组真、假腔主动脉平均强化值比较,差异有显著性意义(P=0.01)。两组图像质量比较,差异在统计学上无显著性意义(P〉0.05)。结论64层螺旋CT团注追踪法主动脉成像能够满足临床诊断主动脉疾病的需要,扫描延迟时间的选择、监测平面的设定以及后续生理盐水的使用,是主动脉CTA检查成功的重要因素。 Objective To investigate the value of 64-slice spiral CT with bolus tracking in aortic angiography. Methods Thirty-nine patients with suspect aortic diseases were evaluated with Siemens 64-slice CT scanner. All of scans were performed with the bolus tracking of 100 ml nonionic contrast material followed by flushing 40-50 ml of physiologic saline solution at 3.5-4 ml/s. Average value of aorta enhancement and its standard deviation over baseline were calculated for both group, with dissection and non-dissection. The imaging qualities in both groups were graded as excellent, good, poor and failure and were compared respectively. Results The mean value of aorta enhancement for non-dissection group was not sig- nificantly different from that in true channel of dissection group (P〉0.05), but significantly different from that in false channel of dissection group (P=0.01) . In dissection group, the mean value of aorta enhancement in true channel differed significantly from that of false channel (P=0. 01). The imaging quality of both groups was not significant different (P〉0.05). Conclusion 64-slice CT angiography with bolus tracking can well display the aortic diseases. It is important to select the proper delay time, tracking planner and solution for aortic angiography.
