ANCA水平检测对判断系统性红斑狼疮活动性和伴肺间质病变的检测意义 被引量:7

The diagnostic value of antineutrophil cytoplasmic antibodies on disease activity and interstitial lung disease in systemic lupus erythematosus

摘要 目的探讨抗中性粒细胞胞质抗体(ANCA)在系统性红斑狼疮(SLE)伴肺间质病变(ILD)中的临床意义。方法用间接免疫荧光法(IIF)及免疫印迹法方法,检测97例SLE患者血清中的ANCA、抗dsDNA抗体、多种自身抗体及补体。采用全胸片及肺高分辨率CT(HRCT)检查,了解97例SLE患者伴ILD的情况。结果①97例SLE患者26例ANCA阳性,均为核周型(pANCA),阳性率为27%。(2)ANCA阳性组患者的抗dsDNA抗体的增高、补体(C3、C4)的下降及血沉(ESR)增高与ANCA阴性组相比,差异均有统计学意义(P〈0.01)。(3)6例ANCA阳性组患者,HRCT显示其中20例伴ILD(77%),71例ANCA阴性组患者,伴ILD30例(42%),ANCA阳性组的ILD发病率明显高于阴性组,两组差异有统计学意义(P〈0.01)。结论ANCA阳性的SLE患者。对判断SLE活动性及伴ILD具有一定的临床价值。 Objective To study the diagnostic value of antineutrophil cytoplasmic antibodies (ANCA) on interstitial lung disease (ILD) in systemic lupus erythematosus. Methods The expression of seral ANCA, anti double-stranded DNA antibodies, other auto-antibodies and complements in 97 lupus patients were tested by indirect immumofluorescence (IIF) and Western blotting. Ninety-seven lupus patients had chest radiography and lung high-resolution computed tomography (HRCT) done to help evaluating the severity of ILD. Results (1)The expression of ANCA in serum of 26 lupus patients was postive (all were perinuclear ANCA) with a posfive rate of 27%. (2)The 26 ANCA-positive patients had higher titers of anti ds-DNA antibodies, lower levels of complements (C3, C4) and higher erythrocyte sedimentation rate compared to ANCA negative patients (P〈 0.01). (3)In those 26 ANCA-positive patients, 20 had ILD (77%). In the other 71 ANCA-negative patients, 30 had ILD (42%). The prevalence of ILD of ANCA-positive patients was higher than ANCA negative patients (P〈 0.01). Conclusion The postive expression of ANCA in lupus patients is related to more active disease and higher frequencey of ILD.
