摘要
目的探讨奥氮平治疗脑外伤后急性谵妄综合征的疗效。方法对76例谵妄病人使用奥氮平治疗并进行前瞻性研究,所有病人随机分为实验组和对照组,使用CCMD-3器质性精神障碍诊断标准和DSM-IV谵妄诊断标准,临床总体印象量表严重程度(CGI-SI)和简明精神病评定量表(BPRS)及GCS昏迷评分标准进行临床评价。结果76例病人中67例进行了完整的评测,其中实验组35例,总有效率85.71%;对照组32例,总有效率37.50%。仅3例出现了恶心、皮疹等副作用,减药后缓解。结论奥氮平能有效控制脑外伤后急性谵妄综合征,同时不影响病人意识的观察,起效快、安全性高。
Objective To study therapeutic efficacy of olanzapine for acute delirium after brain trauma. Methods Seventy-six cases were enrolled in the prospective study. All the patients were randomly divided into the experimental group and control group, Chinese Classification of Mental Disorders Version 3 （CCMD-3） and DSM-Ⅳ were adopted as the diagnostic criteria of delirium, and BPRS, CGI-SI, and GCS were used to assess the clinical effect. Results Among the 76 cases, 67 had the integrated data, 35 were in the experimental group with an overall efficiency of 85.71%, and 32 were in the control group with an overall efficiency of 37.50%. Only 3 cases had such side efficts as nausea and rashes, and the symptoms were relieved after reducing the dose. Conclusion Olanzapine can effectively control acute delirium after brain trauma with a high efficiency and safety, and has no affect on the observation of patients.
