钬激光治疗兔肾VX2肿瘤的实验研究 被引量:1

摘要 目的探讨钬激光在治疗肾实质肿瘤中的应用。方法制作兔的肾VX2肿瘤模型,超声影像学技术初步评估肿瘤组织的大小,在手术直视下,应用不同的钬激光照射参数,对肿瘤组织进行照射。通过肉眼、光镜及图像处理软件,观察钬激光对肿瘤组织的生物学效应及范围。结果钬激光照射后肿瘤组织肉眼观察被完全凝固,分为汽化层、凝固坏死层、外围损伤层三层。HE染色可发现肿瘤细胞发生凝固性坏死,电镜下亦见到不可逆的破坏。切割带外围热损伤的宽度局限于0.4 ̄0.8mm(平均0.54mm)。激光功率与损伤的肿瘤组织大小存在正相关关系。结论钬激光对肾VX2肿瘤组织具有精确的汽化、切割、凝固效果,同时止血可靠,可以通过调整激光的功率控制毁损肿瘤组织的范围,可操作性强。可以用于肾实质肿瘤的临床治疗,也可在影像技术的引导下行穿刺介入治疗,或者通过腹腔镜下治疗,以及应用于保肾手术中的辅助切割。 [Objective] To investigate the effectiveness of Ho:YAG laser irradiation in the treatment of kidney tumors. [Methods] Ten New Zealand rabbits bearing VX2 kidney tumors were irradiated by Ho: YAG laser with certain parameter in vivo. The vaporized incision, coagulation,haemostatic effect and pathologic alteration of laser to tumor tissues were observed and measured. [Results] Tumor ceils underwent coagulation necrosis immediately after irradiation with Ho: YAG. There Were three layers after Ho: YAG laser irradiation (The layer of vaporization, coagulation necrosis and thermal damage). The width of thermal damage and its controls were determined by computer image analysis. Tissues. There was a positive correlation between irradiation power and ablation effect.The control was easily handled. [Conclusion] The experiment indicates that Ho:YAG laser should be effective in the treatment of kidney tumors.
