摘要 目的:本研究通过观察HIF-1α正常乳腺组织及乳腺癌组织中的蛋白表达,探讨HIF-1α在乳腺癌发生、发展中的作用.方法:采用免疫组织化学SP法检测正常乳腺组织(20例)、乳腺囊性增生组织(20例)、乳腺不典型增生组织(20例)及乳腺癌组织(60例)中HIF-1α蛋白的表达,并分析其和乳腺癌临床病理特征的关系.结果:HIF-1α蛋白在正常乳腺组织无阳性表达,在乳腺囊性增生组织、乳腺不典型增生组织及乳腺癌组织中的阳性率分别为5%,30%,61.67%,组间比较差异有统计学意义(P<0.05);低分化组癌组织中HIF-1α蛋白的表达率高于高、中分化组,差异有统计学意义(P<0.05);并且随着分期越高,其阳性表达率越高,差异有统计学意义(P<0.05);淋巴结转移组中的HIF-1α表达明显高于无转移(P<0.05);HIF-1α蛋白表与患者5a生存率有关(P<0.05).结论:HIF-1α蛋白在乳腺癌发生、发展不同阶段的表达呈进行性上升的趋势,过度表达提示预后不良. AIM: To investigate the role of HIF-1α in tumorigenesis and progression of breast carcinoma through observing the expression of HIF-1α in the normal mammary tissue,cystic hyperplasia or atypical hyperplasia mammary tissues and breast carcinoma. METHODS : The expression of HIF-1αt in the normal mammary tissue ( n = 20), cystic hyperplasia mammary tissue ( n = 20), atypical hyperplasia mammary tissue ( n = 20) and breast carcinoma ( n = 60) were evaluated by SP immunohistochemistry. The relationship between HIF-1α expression and the clinicopathological features of breast cancer was assessed. RESULTS: The expression of HIF-lot in the normal mammary tissue was negative, and the positive rates of HIF-1α were 5%, 30% and 61.67% in cystic hyperplasia mammary tissues, atypical hyperplasia mammary tissues and breast carcinoma, respectively ; there was significant difference among groups ( P 〈 0.05 ). In breast carcinoma, the positive expression of HIF-1α in low differentiated grade group was higher than that in high and middle differentiated grade group (P 〈 0.05 ), and it rose with the development of clinical stages (P〈0.05); the positive expression of HIF-1α in lymph node metastasis group was significantly higher that in lymph node metastasis negative group, and it was correlated with 5-year survival rate ( P 〈 0.05 ). CONCLUSION : Overexpression of HIF-1α is common in tumorigenesis and progression of breast carcinoma. Altered expression of HIF-1α may contribute to tumorigenesis and progression of breast carcinoma, and its overexpression indicates worse prognosis.
