摘要
目的评价小剂量果糖对老年腹部手术后患者血糖和胰岛功能的影响。方法将满足方案标准并获得知情同意的大于60岁的老年腹部手术后患者60例随机分成两组,研究组和对照组各30例。术后第1天开始禁食并静脉注射5%果糖注射液或5%葡萄糖注射液500ml,连续3天。观察用药前和用药中血糖、胰岛素和胰岛素C肽的变化,比较两组间的差别。同时观察用药后血尿酸和乳酸的变化。结果按计划完成研究58例,淘汰2例;对照组和研究组在输液前后3天血糖的平均变化值分别为(2.33±2.32)和(0.28±1.59)mmol/L,两组有显著性差异(P<0.01)。在输注果糖中,研究组血胰岛素和胰岛素C肽的变化值分别为(4.2±11.5)μU/ml和(203±279)pmol/L;对照组在输注葡萄糖中血胰岛素和胰岛素C肽的变化值分别是(13.6±15.2)μU/ml和(559±302)pmol/L,对照组升高的幅度明显大于研究组,两组差异有显著意义(P<0.05);两组血乳酸和尿酸结果相似。结论与葡萄糖比较小剂量输注果糖对老年手术后患者血糖和胰岛功能的影响较小。
Objective To evaluate the effects of small dose fructose on serum glucose and pancreas islet function in elder post abdominal operation patients. Method Randomize 60 elder consented post abdominal operation patients who met defined criteria into two groups, with 30 patients in the study group and 30 in the control groups. The subjects in the study group were injected 5% fructose 500 ml, intravenously three days one or two days after operation. Yet the control group were given 5% glucose 500 ml. The changes of serum glucose, insulin （INS） and INS C-peptide （CP）, lactic acid and uric acid, before and after injection were measured. Results We completed 58 cases according to the protocol. The effects of 5% fructose on serum glucose were with （0. 28 ± 1.59） mmol/L changes after the injection. However in control group, the effects relatively mild of 5% glucose mild. Yet in the control group, the two parameters increased remarkably. The differences were significant （P 〈 0. 05）. No effect on serum lactic acid and uric acid was observed in both groups. Conclusion Small dose adequate intravenous injection of fructose has relatively mild effects on serum glucose and pancreas islet function.
