老年性痴呆症患者及转基因小鼠模型海马部位老年斑的特征 被引量:1

摘要 于2002/2004在安徽医科大学附属医院老年病研究所采用老年性痴呆症海马6例,男3例,女3例,年龄60~81岁,死亡耽搁时间小于8h。根据NINCDS-ADRDA标准,结合病史和实验室检查,排除其他原因的痴呆病,诊断为“很可能是老年性痴呆症”。神经病理学诊断按照CERAD标准,海马和颞叶皮质都显示广泛的老年斑和神经原纤维缠结。Braak分级Ⅵ级。老年性痴呆症转基因小鼠11只,11月龄,雄性和雌性分别为5只和6只。采用连续切片法结合改良Bielshowsky银染和免疫组织化学染色分别检测了6例老年性痴呆症患者和11只转基因小鼠的海马部位老年斑。结果显示:①两种染色方法均可清楚的显示各类老年斑。经典斑斑块呈圆形,边界较清晰,核心周围为一透明晕,外环含有淀粉样蛋白及粗细不等的嗜银纤维。弥散斑边界比经典斑模糊,整个斑块由淀粉样蛋白及纤细的网状嗜银结构组成。②经典斑不同切面呈不同形态,而弥散斑形态比较一致。③老年性痴呆症转基因小鼠,未发现经典斑而仅有少量弥散斑,但海马和颞叶皮质突触已明显丢失。免疫荧光染色法特异性最好,背景最低;免疫组织化学染色次之;银染法敏感性较好,但特异性稍差,染色背景最重。 From 2002 to 2004 senile plaques were selected from hippocampus of 6 patients with senile dementia, including 3 males and 3 females, aged from 60-81 years, in Geriatrics Institutes, Affiliated Hospital of Anhui Medical University. Delay time after death was shorter than 8 hours. According to NINCDS-ADRDA standard, they were diagnosed as having senile dementia after other dementia was excluded, combing with the history of disease and laboratory examination. On the basis of CERAD standard, senile plaques and neurofibrillary tangles appeared widely in hippocampus and cortex of temporal lobe. It was Ⅵ grade in Braak rating. Eleven transgenic mice with senile dementia, aged 11 months, 5 males and 6 females, were selected. The senile plaques in hippocampus were measured in 6 patients with senile dementia and 11 transgenic mice with serial section method combined with modified Bielshowsky silver staining and immunohistochemical method, respectively. The classic results showed that ① various senile plaques appeared in the two methods. The classic plaques were round, with clear boundary. There was a transparent areola around the core. Outer ring contained amyloid and argentophilic fibers of different caliber. Diffuse plaque was with obscure boundary compared with the classic plaques. The plaque was composed of amyloid and tenuous mesh argyrophilic structure. ② The different cross-section of classic plaque was different in form, whereas the diffuse plaque was in similar structure. ③ In transgenic mice with senile dementia, there was no classic plaque, but a few diffuse plaques. Distinct loss had appeared in synapsis of hippocampus and cortex of temporal Iobe. Immunofluorescence staining had the best specificity and lightest background, followed by immunohistochemistical staining. Argentation had better sensitivity, but with worse specificity and the darkest staining background.
