摘要
目的从分子水平阐明了慢性肾衰(CRF)早期脂质变化与动脉粥样硬化(AS)发生发展的关系及它们之间的相互作用。方法Wistar鼠5/6肾切除CRF条件形成后,分别在10,20,40天各时相点同时测定主动脉弓组织中甘油三脂(TG)、总胆固醇(T-Ch)、总磷脂(T-PL)的动态水平,并在10、20、30、40天时相点动态观察了血浆中TG、T-Ch、T-PL及红细胞膜T-Ch、T-PL的水平。结果主动脉弓组织中的三种脂质水平从10天时相点开始稍高于同时相点对照组,在20天TG首先明显增高(P<0.05),在40天T-Ch和TG增高非常显著(P<0.001),T-PL明显增高(P<0.05)。血浆中的TG、T-Ch水平与主动脉组织不同程度的升高,两者显著相关。红细胞膜T-Ch升高,T-PL下降导致红细胞刚性增强,变形能力下降。结论揭示CRF早期就存在脂质代谢异常的同时伴随着主动脉脂质沉积和红细胞膜脂质成分的改变。提示CRF早期已经具备了合并心血管病发病的危险因素。
ObjectiveTodeterminetherelationshipandinteractionbetweenthechangesoflipidmetabolismanddevelopmentofatherosclerosisattheearlystageofchronicrenalfailure.MethodsThetriglyceride(TG),totalcholesterol(T-Ch)andtotalphospholipid(T-PL)levelsinaroticarcheswereexamined.WeanalysedtheTGandT-PLlevelsintheplasmaandmembraneofery-throcytesonthe10,20,40daysafter5/6nephrectomyofWistarrats.ResultsThelevelsofthreekindsoflipidsinthearoticarcheswereelevatedslightlyonthe10days.TGlevelinthearoticarcheswaselevatedsignificantlyonthe20days(P<0.05).TG,T-ChandTPLlevelinthearoticarcheswereelevatedsignificantlyonthe40days(P<0.001,P<0.001,P<0.05).TGandT-Chlevelsintheplasmawerealsoelevatedsignificantlyandrelatedtothelevelsinthearoticarches.TheelevatedT-ChanddecreasedT-PLlevelsinthemembraneoferythrocytesresultedinthedecreaseofery-throcytedeformability.ConclusionsTheresultssuggestedthatwhenabnormalityoflipidmetabolismoccures,lipidstrappedintoarotaandcomponentsoferythrocytemembranealterattheearlystageofchronicrenalfailure,indicatingthattheriskfactorsofcardiovasculardiseasesexistattheearlystageofchronicrenalfailure.Thestudyprovidedatheoreticalbasisfortheearlypreventionandtreatmentofcardiovascularcomplicationsofchronicrenalfailure.
