摘要
目的:探讨精浆热休克蛋白70及细胞因子表达对慢性前列腺炎分型诊断的临床价值。方法:采用酶联免疫吸附测定法(EL ISA)对慢性细菌性前列腺炎(Ⅱ)40例,炎症型慢性非细菌性前列腺炎(ⅢA型)30例,非炎症型慢性非细菌性前列腺炎(ⅢB型)30例患者及30例正常对照组精浆中热休克蛋白70及肿瘤坏死因子-α,白细胞介素-2,白细胞介素-10含量进行测定,并结合慢性前列腺炎症状评分进行相关性研究。结果:Ⅱ型患者与正常对照组比较,热休克蛋白和肿瘤坏死因子-α含量显著升高(P>0.05),白细胞介素-2及白细胞介素-10含量显著降低(P<0.01)。与ⅢA型、ⅢB型组比较,差异均有显著性(P<0.01),ⅢA型与ⅢB型组比较,差异无显著性(P>0.05)。热休克蛋白70和肿瘤坏死因子-α与前列腺压出液(EPS)中W BC计数呈正相关;与症状评分呈负相关。而白细胞介素-2及白细胞介素-10与EPS中W BC计数无相关性,与症状评分呈正相关。结论:精浆中热休克蛋白70及肿瘤坏死因子-α,白细胞介素-2,白细胞介素-10在慢性前列腺炎的发病过程中起重要作用,对其分型诊断具有一定的临床价值。
Objective To study the expression of heat shock protein 70 and cytokines in the seminal plasma on the clinical typing of chronic prostatitis patients. Methods The levels of heat chock protein 70 and the cytokines in the seminal plasma of 40 patients with chronic bacterial prostatitis (type Ⅱ), 30 patients with chronic inflammatory non-bacterial prostatitis (type Ⅱ A), 30 patients with chronic non-inflammatory non-baterial proslatitis (type ⅡB), and 30 normal controls were measured with ELISA. The results were analyzed with chronic prostatitis symptoms index. Results The heat shock protein, TNF-α levels in type Ⅱ chronic prostatitis patients were significantly higher (P〈0. 05) and the IL-2 and IL-10 levels were significantly lower(P〈0. 01) than those in the controls, type Ⅱ A and type Ⅱ B chronic prostatitis patients (P〈0. 01). There was no statistic difference in the heat shock protein 70 and cytokines between type ⅡA and type ⅡB chronic prostatitis patients (P〉0.05). The heat shock protein 70 and TNF-α levels were positively correlated with WBC counts and negatively correlated with NIH-CPSI scores. The IL-2, IL-10 levels were positively correlated with NIH-CPSI scores and were not correlated with WBC counts. Conclusion The heat shock protein 70 and cytokines may play an important role in pathogenesis of prostatitis, and achieve a valuable effect on the clinical typing of chronic prostatitis.
