摘要
目的:研究甲状腺肿瘤的超声图像与病理组织类型的关系,探讨高频超声对甲状腺肿瘤的鉴别诊断价值。方法:回顾性分析经超声检测的93例甲状腺肿瘤病灶的声像图特征,并与其病理组织学类型比较、分类,提出鉴别要点。结果:93例经手术病理证实的甲状腺肿瘤病灶的声像图特征分为:实质性低回声21例,稍强回声13例,混合回声52例,囊性为主回声7例,微粒状强回声钙化点5例。甲状腺肿瘤超声诊断与手术病理结果符合率为82.68%(77/93)。其中甲状腺腺瘤的诊断符合率为87.5%(56/64),甲状腺癌的诊断符合率为46.1%(6/13)。结论:甲状腺肿瘤超声特征分类与其病理结构相关。高频超声对甲状腺良性肿瘤的诊断符合率高,具有较高的诊断与鉴别诊断价值,但对甲状腺癌的诊断缺乏特异性,尤其对甲状腺微小癌、隐匿性新生物的诊断与鉴别诊断有待进一步研究。
Objective： To make a study of relation between ultrasonic graph and pathologic organisms, and to explore differential diagnostic value of high frequency ultrasound in respect of thyroid tumor. Methods： Carrying out retrospective analysis on sonographic features of 93 cases of thyroid tumor focus through ultrasonic test, making comparison with their types of pathologic organism, classifying and proposing differentiating points. Results. The sonographic features of 93 cases of thyroid tumor focus proved by operational pathology were divided into： 21 cases of substantial low echo, 13 cases of a bit stronger echo, 52 cases of mixed echo, 7 cases of dominantly cystic echo, and 5 cases of granular calcified spots with strong echo. The coincidence rate between the ultrasonic diagnosis on thyroid tumor and the result of operational pathology was 82. 68 % （77/93）, of which the diagnostic coincidence rate of thyroid adenoma was 87.5 % （56/64） and that of thyroid cancer was 46.1% （6/13）. Conclusion： The classification of ultrasonic features of thyroid concerns its pathologic structure. High frequency ultrasound contributes high diagnostic coincidence rate in respect of benign tumor of thyroid, is provided with relatively higher value of diagnosis and differential diagnosis, but is short of specificity in diagnosis on thyroid cancer, especially requires further research in respect of diagnosis and differential diagnosis on thyroid slight cancer and occult neoplasm.
