摘要
目的评价和分析次极量平板运动试验(TET)对冠心病的诊断的价值。方法以冠状动脉造影(CAG)为诊断冠心病的标准,对115例患者同时行冠状动脉造影和平板运动试验检查,对其结果进行对比,分析平板运动试验的敏感度和特异度等指标。结果平板运动试验阳性者72例,冠状动脉造影检查确诊冠心病60例,平板运动试验诊断冠心病灵敏度为82.2%,特异度为71.4%,符合率为78.3%,阳性预测值为83.3%,阴性预测值为69.8%。TET对冠状动脉多支病变的检出率明显高于单支病变(P<0.05)。结论平板运动试验是一种诊断冠心病较理想的、安全的无创诊断方法,其敏感度和特异度较高,尤其对多支血管病变患者。
[Objective] To evaluate the diagnostic value of treadmill exercise testing （TET）. [Methods] 115 patients with suspicious coronary artery disease（CAD） were examined by TET and coronary angiography（CAG） and the diagnostic results were compared and determined. [Results] Among 72 patients with positive diagnosis by TET, there were 60 ones with positive diagnosis, the sensitivity was 82.2%, the specificity was 71.4%, the diagnostic accordance rate was 78.3%, the positive and negative predictive accuracy was 83.3% and 69.8% respectively, There was also higher detectable rate for TET in the multiple vessel disease group compared with the single vessel disease group （P 〈0.05）. [Conclusion] Exercise treadmill testing is an effective non-invasive method for diagnosis of CAD with high sensitivity and speciality. Some parameters in TET are helpful to predict the severity of CAD.
