摘要 目的探讨早期肠内营养(EN)在老年危重病人中的临床应用。方法85例老年危重病患者,随机分为入院后1周内开始实施EN的45例为EN组;行完全胃肠外营养的40例为对照组,并分别在营养支持前后测肝肾功能、血糖、电解质、血清蛋白、血红蛋白、免疫球蛋白及淋巴细胞计数等。结果EN组血清白蛋白、前白蛋白、转铁蛋白营养支持后显著增高(P<0·05),免疫球蛋白和淋巴细胞计数在营养支持后明显升高(P<0·01)。结论对危重病人进行早期肠内营养支持是可行、安全和有效的。能改善危重病人的营养状况及免疫功能。 Objective To analyze the application of early enteral nutrition (EN) in critically ill patients. Methods Forty-five elder patients undergoing EN within one week after hospitalization, and 40 cases undergoing total parenteral nutrition (TPN)were chosen in this study. The renal and liver functions, serum glucose, electrolytes, plasma protein, Hb, immunoglobulin and lymphocyte count were measured before and after nutrition support. Results The levels of serum albumin, prealbumin, transferring irnmunoglobulin and lymphocyte count were significantly elevated after nutritional support compared with those before in the EN patients (P〈0.05 or P〈0. 01). Conclusion The early enteral nutrition is safe, efficient and feasible in critically ill elder patients, which can improve the nutrition, immune function and prognosis.
