摘要
目的探讨彩色多普勒超声在颈动脉、椎动脉、锁骨下动脉、无名动脉、肾动脉、髂动脉支架植入术后监测中的价值。方法以高、低频探头观察31例因外周、腹腔动脉狭窄性病变、动静脉瘘行动脉裸支架或覆膜支架植入患者(植入37个支架)的支架有无术后再狭窄或闭塞。结果在二维声像图上不易分辨裸支架和覆膜支架。彩色多普勒超声可发现双侧肾动脉起始段支架植入、锁骨下动脉起始部支架植入术后再狭窄和椎动脉起始部植入支架阻塞各1例,颈动脉支架植入术后狭窄3例,支架植入术后再狭窄率为16.2%(6/37),支架植入术后阻塞率为2.7%(1/37)。结论彩色多普勒超声可作为颈动脉、椎动脉、锁骨下动脉、无名动脉、肾动脉、髂动脉支架植入术后监测的首选影像学手段。
Objective To explore the value of color Doppler ultrasonography (CDU)as a postoperative monitoring in patients with stents implantation in carotid, vertebral, subclavin, innominate or iliac arteries. Methods Thirty-one patients (thirty-seven stents) with stenosis or arteriovenous fistula of carotid artery, vertebral artery,subclavical artery,innominate artery, renal artery, arteria iliaca were observed with high or low frequency ultrasonic transducer before and after implant bare or membrane covered stents. Results Two dimensional ultrasound was difficult to differentiate the type of stnets. Two stenotic stents in initial segment of both renal arteries,one stenotic stents in inital segment of subclavical artery,one occlusive stent in vertebral artery, three stenotic stents in carotid arteries were found. The restenosis rate of stent implantation was 16.2% (6/37),and the occlusive rate of stent implantation was 2.7% (1/37). Conclusions CDU was an effective imaging technique in postoperative trace of great artery stents such as carotid artery, subclavical artery, innominate artery, renal artery, arteria iliaca. In addition, postoperative stenosis and occlusion in bare or covered stents can not be discerned clearly with two dimensional ultrasound.
