心电图T波改变对急性下壁心肌梗死患者梗死相关血管判断临床价值

The value of electrocardiography T-wave changes in identifying the infarct-related artery in patients with acute inferior myocardial infarction

摘要 目的:探讨心电图(ECG)T波改变对急性下壁心肌梗死患者梗死相关血管(IRA)判断的价值。方法:选择2002年7月至2005年2月间的急性下壁心肌梗死患者177例,回顾性分析其症状发作后6h内ECG改变。结果:T波振幅Ⅲ导联>Ⅱ导联和V4R导联T波直立或正双向判断梗死相关血管(IRA),为右冠状动脉(RCA)的敏感性是88%和90%,特异性是84%和89%。结论:急性下壁心肌梗死早期,T波振幅Ⅲ导联>Ⅱ导联和V4R导联T波直立或正双向是判断IRA为RCA的新标准。 Objective: To investigate the Electrocardiography (ECG) T-wave changes for identifying the infarct-related artery (IRA) in patients with acute inferior myocardial infarction (MI). Methods: We retrospectively studied 177 patients with acute inferior MI hospitalized in the cardiovascular department of 3 hospitals. The ECG recorded within 6 h of the onset of symptoms were analyzed. Results: A T-wave amplitude that is greater in lead Ⅲ than in lead Ⅱ and an upright or positive biphasic T-wave in lead V4R had a sensitivity of 88% and 90% ,a specificity of 84% and 89% respectively ,for identifying Right Coronary Artery (RCA) as the IRA. Conclusions A T-wave amplitude that is greater in lead Ⅲ than in lead Ⅱ and an upright or positive biphasic T-wave in lead V4R is a new criteria for identifying RCA as IRA in patients with acute inferior MI.
