摘要
目的:探讨人工流产与自然流产蜕膜组织中凋亡调控基因的表达。方法:人工流产和反复自然流产蜕膜组织各30份,应用免疫组织化学方法测定Bc l-2、Bax、P 53、Fas、FasL 5种调控基因在蜕膜组织中的蛋白表达。结果:Bc l-2的表达在人工流产组与自然流产组无明显差异(P>0.05);Bax在自然流产组中的表达较人工流产组明显增强(P<0.05);FasL的表达在自然流产组明显低于人工流产组(P<0.05);Fas表达自然流产组与人工流产组无明显差异(P>0.05);P 53蛋白在自然流产组的表达强于人工流产组(P<0.05)。结论:自然流产的发生有3条凋亡调控途径:一为Fas/FasL途径:由于滋养细胞表面FasL表达的减少,不能引起母体内特异性的活化T细胞的凋亡,导致母体对胚胎的排斥反应,引起自然流产;另一为Bc l-2/Bax途径:虽然Bc l-2的表达在人工流产与自然流产蜕膜中无明显差异,但由于Bax在早孕自然流产蜕膜组织中的表达明显增强,导致Bc l-2/Bax的表达量失衡而引起流产;再一为P 53途径:野生型的P 53增多诱发细胞凋亡而导致流产发生。
Objective: To question the expression of apoptosis control gene in deciduas of spontaneous abortion and artificial abortion. Methods: Deciduas tissue from 15 cases of recurrent spontaneous abortion and30 cases of artificial abortion, to assay the protein expression of Bcl -2, Bax, P53, Fas, FasL in deciduas by immunohistochemistry. Results: expression of Bcl -2 has not obviously difference in spontaneous abortion group and artificial abortion group ( P 〉 0. 05 ) ; expression of Bax was obviously strong in spontaneous abortion group ( P 〈 0. 05 ) ; expression of FasL was obviously weaker in spontaneous abortion group than artificial abortion group ( P 〈 0. 05 ) ; expression of Fas has not obviously difference in spontaneous abortion group and artificial abortion group ( P 〉 0. 05 ) ; expression of P53 was obviously strong in spontaneous abortion group ( P 〈 0. 05 ) . ConclusiOn: Three apoptosis control pathway correlated to spontaneous abortion , the first is Fas/FasL pathway: the expression of FasL location in trophoblast surface decrease, dont make maternal specific activation T cell apoptosis, to lead to reject reaction and spontaneous abortion; the second is Bcl- 2/Bax pathway: although expression of Bcl -2 has not obviously difference in spontaneous abortion group and artificial abortion group, expression of Bax has obviously reinforcement in spontaneous abortion group, lead to Disequilibrium of the expression of Bcl - 2/Bax and abortion , the third is P53 pathway : the increasing of wild type P53 induce apoptosis and spontaneous abortion.
