摘要 目的:探讨用新生儿足跟全血血片TSH筛查新生儿先天性甲状腺功能减低症(CH)的切值和监测人群碘营养状况的可能性。方法:用TRFIA法筛查新生儿72h足跟全血血片TSH。结果:正常新生儿TSH频数呈正偏态分布,TSH〉5mU/L比率2.17%,99%分位数为6.89μU/ml,筛查确诊17例CH,其中5例(29.4%)TSH筛查时呈现低值9.11~18.0μU/ml。结论:本地区筛查CH切值以8μU/ml为宜,TRFIA筛查新生儿TSH〉5mU/L比例〈3%可作为人群碘营养状况正常的标志。 Objective : To study the applicability of the CH cut - off by neonatal screening and of the monitoring of peopleg normal nutritional condition (TSH 〉 5mu/L percentage 〈 3% ) . Methods: Screen the TSH of full heel blood of neonate of 72h's old by TRFIA method. Results: the TSH frequency of normal neonate shows positively skew distribution (TSH 〉 5mu/L percentage 2. 17% , 99% quartile: 6.89 μU/ml) . 5 samples among 17 samples (29.4%) diagnosed to be CH by screening shows low level 9. 11 - 18.0 μU/ml when being screened. Conclusion: It's appropriate to set the CH cut -off of local area at 8 μU/ml TSH 〉 5 mu/L and percentage 〈 3% by TRFIA neonatal screening method can be regarded as people's normal iodic nutritional level.
