超声引导周围神经阻滞10年来已经日益普及。超声成像技术给麻醉科医师提供了一个能够直视周围神经、神经周围的解剖结构、穿刺针的置入和观察局麻药注射的非常有用的工具。超声引导周围神经阻滞代表了区域阻滞麻醉的新进展。更为重要的是,与传统神经阻滞相比此法提高了阻滞的安全性、改善了阻滞效果和患者的舒适性。本文介绍了医用超声成像的基本原理以及超声引导周围神经阻滞的优点,同时介绍了用于周围神经阻滞的超声仪器、临床技术以及常用周围神经阻滞的超声图像。
Ultrasound guidance in peripheral nerve blocks (PNB) has become increasingly popular for the past decade. Ultrasound imaging provides a valuable tool for anesthesiologists to directly visualize the peripheral nerves and surrounding anatomical structures, placement of needles, and injection of local anesthetics. Ultrasound guidance in PNB represents a new advancement and development in regional anesthesia. More importantly, it intuitively improves safety, eficacy, and patient comfort compared with traditional nerve localization techniques. This article reviewed principles of medical ultrasound and advantages of ultrasound guided PNB over traditional nerve localization techniques. It also introduced ultrasound equipment used for nerve blocks, clinical techniques as well as sonographic images of the peripheral nerves which are commonly blocked.
