摘要
目的探讨胸大肌肌皮瓣行口底癌切除舌再造术的术后护理经验。方法总结7例行胸大肌肌皮瓣舌再造术患者的护理观察重点,包括皮瓣的颜色、肿胀程度、毛细血管反应及皮瓣蒂根部血运情况;再造舌体伤口周围皮肤及渗出物的性状;而呼吸道的观察在于患者的呼吸和血氧监护。结果7例移植皮瓣均成活,再造舌功能恢复良好,其中1例术后4h肌皮瓣颜色变白,经及时处理后缓解。无一例出现伤口感染和呼吸道并发症。结论胸大肌肌皮瓣行舌再造术后护理观察的重点在于皮瓣、口腔伤口和呼吸道观察,细致、全面的护理观察有助于采取有针对性的护理措施,降低术后并发症。
Objective To study the postoperative nursing experiences about the tongue reconstruction with pectoralis major myocutaneous flap. Methods Summarized postoperative nursing experiences about the tongue reconstruction with pectoralis major myocutaneous flap for 7 patients. The nursing points included： the observation about the color of myocutaneous flap, the oozing of blood, the tumefaction, the respiratory tract nursing, the wound nursing, and so on. Results All the myocutaneous flap were survived, the function of tongues which have reconstructed were good. There were no complications occurred. Conclusion The key nursing points were the myocutancons flap nursing, wound nursing and the respiratory tract nursing, which can effective guarantee the survival of the myocutaneous flap.
