摘要
应用间接免疫荧光及免疫印迹技术检测65例PM、DM、MCTD和PM-Scl,检出ANA,ASA及7种ENA多肽抗体,其阳性率为86.2%。ENA多肽抗体中MCTD抗RNP抗体阳性率最高(87.5%),PM抗Jo-1抗体次之(28.6%),DM抗Jo-1抗体阳性率较低(10%)。抗Jo-1抗体主要存在于PM,可出现于疾病早期并持续存在于整个病程。抗Jo-1抗体滴度与CK水平和疾病的严重程度有关,病情加重常伴随抗体滴度增加。某些微小病毒基因组的tRNA样结构与宿主Jo-1抗原——组氨酰tRNA合成酶的部分结构相似,并能特异地被合成酶氨基酰化,形成免疫原性复合物,诱发自身免疫反应。
The serum autoantibodies of 65 patients with polymyositis (PM), dermatomyositis (DM), mixed connective tissue(PM-Scl)were measured with indirect immunofluorescence method and im-munoblotting technique. The AN A, ASA and seven EN A polypeptide antibodies were detected and their positive rate was 86. 2%- The positive rate of anti-RNP antibody in MCTD was 87. 5%. The positive rate of anti-Jo-1 antibody in PM and DM was 28. 6%and 10% respectively. The anti-Jo-1 antibody was specifically found in PM and may appear in early period,and usually remain detectable in all course of the disease. Anti-Jo-1 liter correlated with CK level and the disease activity,its increase might predict the exacerbation. The tRNA-like structures on picornaviral genomes were similar to some structures of host Jo-1 antigen-histidyl-tRNA synthetase, which can be specifically aminoacylated by the synthetases,forming immunogenic complexes and triggering the autoimmune reaction.
