初发2型糖尿病患者血清LTB_4与PAF-AH改变的意义研究 被引量:4

摘要 目的探讨初发2型糖尿病患者外周血中白三烯B4(LTB4)与血小板活化因子乙酰水解酶(PAF-AH)活性变化的临床意义。方法用ELISA法检测55例初发的2型糖尿病患者血清中LTB4含量与PAF-AH活性,与25例健康体检者对照,并比较它们与患者空腹血糖、HbAlc和空腹胰岛素之间的关系。结果2型糖尿病患者血清中LTB4含量(864.69±202.67)ng/L与健康对照组(353.31±120.93)ng/L比较,差异有显著性(P<0.01);而2型糖尿病患者血清PAF-AH活性(9.36±3.14)!mol/min·L与健康对照组(6.11±1.23)!mol/min·L比较有显著性差异(P<0.01)。2型糖尿病空腹血糖FBG(12.54±4.94)mmol/L、HbAlc(10.23±3.13)%和空腹胰岛素FINS(10.9±11.1)mU/L与外周血LTB4含量和PAF-AH活性之间均呈明显的正相关性(P均<0.01),患者外周血LTB4含量与PAF-AH活性之间也呈明显的正相关性(P<0.01)。结论2型糖尿病患者存在炎症反应,血清LTB4与PAF-AH活性明显升高,提示它们可能共同参与了初发2型糖尿病的病理生理过程,可作为初发2型糖尿病患者病情变化的监测指标之一。 Objective To evaluate the clinical significance of serum leukotriene B4 (LTB4) and the activity of platelet-activiting factor acetylhydrolase (PAF-AH) in patients with T2DM diagnosed for the first time. Method The levels of serum LTB4 and the activity of PAF-AH in 55 patients with T2DM and 25 healthy subjects were measured by the ELISA method. The serum levels of FBG, FINS and HbAlc were also analyzed. Result The level of serum LTB4 in T2DM patients (864.69±202.67)ng/L was higher than those in the control group (353.31±120.93)ng/L, P〈 0.01. The activity of PAF-AH in T2DM patients (9.36±3.14)μmol/min·L was significantly higher than those of control group (6.11±1.23)μmol/min·L, P〈 0.01. In T2DM patients, the levels of LTB4 and PAF-AH activity were positivey correlated with the serum levels of FBC(12.54±4.94)mmol/L, FINS (10.9±11.1)mU/L, and HbAlc (10.23±3.13)%. Conclusion Inflammation is frequently observed in patient with T2DM. The levels of serum LTB4 and the activity of PAF-AH in T2DM patients were increased. These observations suggested that the two inflammatory mediators may play an important role in the pathogenesis of the inflammation. Both LTB4 and PAF-AH should be measured when the patient with T2DM was examined.
