摘要
目的:探讨阴道特制的塑料管M cC artney管在完全性腹腔镜全子宫切除术的作用、手术要点、适应证和临床应用价值。方法:对68例因各种妇科良性疾病而需行全子宫切除术的患者,采用完全性腹腔镜全子宫切除术,术中使用阴道M cC artney管,观察其手术时间、术中出血量和术中术后并发症。结果:67例成功地完成完全性腹腔镜全子宫切除术,有1例因出血而中转开腹。平均手术时间130m in,平均出血量100 mL。有1例术后出现膀胱阴道瘘。结论:M cC artney管的应用可使完全性腹腔镜全子宫切除术易行,提高手术的安全性。
Objective To evaluate the operative techniques, indications and clinical value of total laparoscopic hysterectomy with McCartney tube. Methods Sixty-eight patients with benign gynecological abnormalities underwent total laparoscopic hysterectomy with McCartney tube. Operative time, blood loss, intra- and post- operative complications were studied. Results Sixty-seven cases of total laparoscopic hysterectomy with McCartney tube were completed successfully. One case was switched to laparotomy because of bleeding. Mean operative time was 130 minutes and average blood loss was 100 mL. There was a postoperative vesico-vaginal fistula. Conclusion Total laparoscopic hysterectomy is feasible and safe. The use of McCartney tube can facilitate total laparoscopic hysterectomy and increase operative safety.
