摘要
目的探讨心力衰竭患者(CHF)左心室心肌非同步运动与二尖瓣反流的关系。方法取标准心尖位左室长轴观、两腔观和四腔观,获得正常人和CHF患者各30例左心室18个节段的定量组织速度成像(QTVI)曲线,测量各节段自QRS波起始至心肌收缩期峰值速度和舒张早期峰值速度的时限Ts和Te,并计算反映左心室非同步运动程度的指标,即18个心肌节段Ts和Te的最大差值(M ax-△Ts和M ax-△Te)。测量并计算二尖瓣反流面积与左心房比(MRA/LAA)。结果CHF组M ax-△Ts和M ax-△Te较正常组明显延长,差异有统计学意义(P<0.05);CHF组MRA/LAA与M ax-△Ts无相关性,与M ax-△Te呈正相关。结论CHF患者左心室存在非同步收缩与舒张,舒张期不同步与二尖瓣反流关系密切。
Objective To study the correlation between left ventricular asynchronicity and mitral regurgitation in patients with congestive heart failure. Methods Quantitative tissue velocity imaging curves of 18 segments were got in 30 cases of congestive heart failure and 30 health controls from standard long - axis, two - chamber, and four - chamber view to measure Ts and Te of each segment from QRS to the peak systolic flow velocity and early diastolic flow velocity and calculate the indices reflecting the degrees of left ventricular asynchronieity, that was the maximum equation of Ts and Te (Max- △Ts and Max - △Te) in 18 myocardial segments, as well as to calculate the ratio of mitral regurgitation area to left atrium(MRA/LAA). Results Max - △Ts and Max - △Te were obviously prolonged in congestive heart failure than in controls ( P 〈 0.05 ). MRA/LAA was not related to Max - △Ts and was positively related to Max - △Te. Conclusion There is left ventricular systolic and diastolic asynchronicity in the patients with congestive heart failure. Diastolic asynchronicity is closely related to mitral regurgitation.
