摘要
目的探讨儿童重症病毒性脑炎(VE)低钠血症与血浆脑利钠肽(BNP)相关性及对病情发展的影响。方法收集2004年3月-2007年5月在滨州市中心医院确诊的74例儿童重症病毒性脑炎的,临床资料。动态观察22例低钠血症Ⅶ患儿病程第1天、第3天及第7天时血浆BNP和血钠水平,并进行相关性研究。同时与血钠正常的52例Ⅶ患儿及血钠正常非病毒性脑炎患儿18例(对照组)进行比较。结果病程第1天时VE组BNP显著高于对照组(P〈0.01)第3天时,血钠正常的Ⅶ患儿与对照组之间已无明显统计学差异,而低钠血症Ⅶ组明显高于血钠正常Ⅶ组(P〈0.01)低钠血症的三个时间段BNP之间差异具有显著性(P〈0.01);低钠血症组各时间段的血钠浓度呈递减,依次比较差异有显著性。低钠血症VE组BNP水平与血钠离子在各时间段具有负相关性。结论BNP参与和导致了Ⅶ低钠血症,并直接经过低钠介导在VE病理过程中起重要作用。
Objective To investigate the correlation between serous brain natriuretic peptide (BNP) and hyponatremia in the children with critical viral encephalitis(VE). Methods Seventy-four children with critical VE were divided into two groups: hyponatremia group(22 cases) and normal natrium group(52 cases), other 18 children with non-VE and normal blood natrium as the control.The concentration of BNP natrium in blood plasma were measured at lst,3rd and7th day in duration of disease. All the results of 3 groups were analysed and compared with each other. Results At the first day the concentration of BNP in the children with critical VE was remarkably higher than that in the control group (P〈0.01), 3rd day the BNP had droped in the cases with VE group of normal blood natrium and without difference between them and the control group, but the hopernatremia group was still higher than normal natrium group and control group (P〈0.01), 7th day the BNP remained high level and the blood natrium was lower in the hyponatremia group, so they had a negative relationship in this group each period. conclusion The change of BNP might take part in the pathological course of hyponatremia in children with. critical VE.
