摘要
目的探讨小儿支原体肺炎外周血细胞和肺部影像学变化,为鉴别诊断提供依据。方法回顾性统计84例确诊为支原体肺炎患儿的外周血细胞、肺部影像学资料。结果婴幼儿组白细胞总数(WBC)增高占55.6%,正常占44.4%,WBC正常、中性粒细胞+单核细胞比例增高占25%,胸片呈弥漫双肺点片状影多见,部分局限在下肺。学龄前儿童+学龄儿童组WBC正常、中性粒细胞+单核细胞比例增高占87.5%,胸片病灶在下肺多见,右肺多于左肺。结论支原体肺炎患儿外周血细胞及肺部影像学改变随年龄不同而异,婴幼儿组WBC及胸片改变多样、无规律性,学龄前儿童+学龄儿童组外周血WBC正常、中性粒细胞+单核细胞比例增高多见,胸片以下肺点片状影多见。
Objective To study the changes of pulmonary iconography and peripheral hemocytes in children with mycoplasma pneumonia in order to offer evidences for its differential diagnosis. Method The data of pulmonary iconography and peripheral hemocytes in 84 cases who were definitively diagnosed as mycoplasma pneumonia patients were retrospectively analysed. Results In the infant group, the total of leucocytes increased in 55.6% of patients,remained normal in 44.4% of patients. In 25% of patients, the total of leucocytes was normal while the ratio of neutrophile granulocyte ＋ hyaline leukocyte increased, most of whose chest radiograph showed diffused spot film shade, some of which was limited in lower lobe of lung. In preschool ＋ school child groups, 87.5% of patients have normal total of leucocytes and high ratio of neutrophile granulocyte ＋ hyaline leukocyte,most of whose chest radiograph showed that the foci were common in lower lobe of lung and right lung. Conclusions The changes of pulmonary iconography and peripheral leucocytes were different with age-specific in children with mycoplasma pneumonia. In the infant group, the changes of leucocytes and chest radiograph are irregular. In the preschool ＋ school child groups, it is common that the total of leucocytes is normal but the ratio of neutrophile granulocyte ＋ hyaline leukocyte increases, The chest radiograph often show spot film shade in lower lobe of lung.
