小剂量瑞芬太尼在硬膜外麻醉下前列腺电切术中的应用 被引量:1

摘要 目的观察小剂量瑞芬太尼用于硬膜外麻醉下前列腺电切术的镇痛效应。方法选择前列腺电切术60例,随机分为两组,A组为瑞芬太尼组(n=30),B组为对照组(n=30),两组均选择L2-3行椎管穿刺头向插管硬膜外麻醉,局麻药选用2.0%利多卡因溶液;A组待麻醉效果确切后,微泵输注瑞芬太尼0.07μg/(kg·min),术毕停用瑞芬太尼。观察两组患者术中血流动力学变化,镇静、镇痛效果及不良反应情况。结果两组患者在硬膜外首量后、手术开始置镜时心率、血压比较有显著差异(P<0.01),而瑞芬太尼组心率、血压波动较小。两组的口述描绘四级评分(VDS)比较也有差异(P<0.05)。结论瑞芬太尼微泵静脉输注0.07μg/(kg·min),用于硬膜外麻醉下前列腺电切术镇痛效果良好,能保持患者术中血流动力学的稳定,且能术中唤醒。
