子宫内膜骨桥蛋白mRNA表达与不明原因不孕的关系 被引量:7

摘要 目的检测骨桥蛋白和整合素β3在正常人子宫内膜mRNA水平的变化,探讨其在不明原因不孕发病机制中的作用.方法采用逆转录-聚合酶链式反应(RT-PCR)对65例分别取自正常月经周期第6~27天的内膜、妊娠45~56天的蜕膜及20例不明原因不孕患者种植窗内膜骨桥蛋白和整合素β3mRNA的表达进行半定量检测.结果骨桥蛋白在增生期、分泌早中晚期和妊娠早期均表达,且妊娠早期的表达较分泌中晚期增强(P<0.05);与正常妇女相比,不明原因不孕患者内膜骨桥蛋白的表达减弱(P<0.05);整合素β3在增生期和分泌早期不表达,分泌中晚期和妊娠早期均表达,且妊娠早期的表达较分泌中晚期显著增强(P<0.01);与正常妇女相比,不孕患者内膜整合素β3的表达明显减弱(P<0.01).结论骨桥蛋白和整合素β3均参与了子宫内膜容受性的建立;不明原因不孕患者种植窗内膜二者均表达减弱,比例失调,可能通过影响内膜的蜕膜化而致内膜容受性低下,是其不孕的原因之一. Objective To investigate the expression of osteopontin (OPN) and integrin β3 at mRNA level in normal cycling and early pregnant endometrium, and explore their roles in infertility of unknown reason. Methods Reverse Transcription polymerase Chain Reaction (RT-PCR) was employed to semi-quantitatively detect endometrial mRNA expression of osteopontin and integrin β3 in 65 samples throughout 6 - 27^th day' s endometrium of menstrual cycle and 45 - 56^th day' s decidua in early pregnancy, and also in 20 samples of the endometrium during the implantation window in unexplained infertile women. Results Osteopontin was expressed in all phases of endometrium, and its normalized intensity increased in pregnant ednometrium Compared with that of the mid-luteal phase ( P 〈 0.05). And as for the relative levels in the mid-secrectory phase, the infertile group was significantly lower than that of normal group (P 〈 0.05). However, integrin β3 only expressed in the secretory and and pregnant phases of endometrium but not expressed in proliferative and early secretory phase. Also, the expression of integrin β3 significantly increased in early pregnant phase when compared with secretory phase (P 〈 0.01 ); and its expression decreased obviously in unexplained infertile women than in the normal (P 〈 0.01 ). Conclusions Both osteopontin (OPN) and integrin β3 are involved in the establishment of the endometrial reeeptivity. And the expression of the two both decreased and imbalaneed in the window phase of endometrial implantation in unexplained infertile women, which may cause low receptivity by dffecting the decidualization of the endometrium and lead to infertility ultimately.
